Overview: Care Coordinator, Urgent Care Hub

This position plays a pivotal role in Heritage Behavioral Health Center's prioritization of providing services to anyone, anytime, and anywhere. Within our Behavioral Health Urgent Care Hub, an individual in this role would provide warm, hospitable contacts with individuals seeking services or support, followed by connection to the supports that individual is seeking. Successful candidates in this role would exemplify flexibility and creativity in promptly connecting individuals with what they are asking for (e.g. food, clothing, transportation, support, services).

Core Responsibilities Include:

  1. Provide warm, hospitable welcome to individuals seeking services/support.
  2. Complete needs assessments to identify needs, including but not limited to mental health, substance use, health, dental, vision, support systems, legal, education, employment, access to transportation or any other services that assist an individual in improving one's quality of life.
  3. Coordinate and actively participate in the referral and linkage to all healthcare and behavioral healthcare needs.
  4. Educate clients and their families on available community resources and assist with making referrals (e.g. Local food panties, arranging medical transportation for appointments, identifying shelters). May accompany client on case-by-case basis.
  5. Assist in engaging and linking individuals to accessing eligible transportation options.
  6. In partnership with the individual, create a crisis safety plan.
  7. Complete assessments and treatment plans using various screening/assessment tools including but not limited to suicide risk assessments, IM+CANS, IM-CAT, and Dimensional Assessment.
  8. Maintain accurate and timely documentation of services provided.
  9. Attend trainings and meetings as required.
  10. Provide other duties as assigned, required, or specifically negotiated with the supervisor

Knowledge, Skills and Abilities:

  1. Ability to work with individuals with a diagnosis of mental health and/or substance use in a kind, caring, and supportive approach that instills hope
  2. Ability to meet people where they are at in their recovery, non-judgmental, and ability to build rapport quickly
  3. Knowledge of healthcare and community resources, and/or the ability to research/find
  4. Ability to work independently and as a team player
  5. Strong customer service skills
  6. Ability to provide feedback and advocacy for client and program needs
  7. Ability to navigate electronic health records
  8. Demonstrated above average computer skills; proficient in Microsoft Word and Excel
  9. The ability to lift 10 pounds is required.

Education and Experience:

  1. Experience working with individuals in behavioral health settings by providing an individualized, strength-based approach to support an individual's wellness and recovery is preferred.
  2. Credential eligible for this position include Mental Health Professional as defined as:
    • Five years' experience in behavioral health,
    • Certified Recovery Support Specialist (CRSS),
    • Bachelors Degree in psychology, social work, human services or related field,
    • Bachelor's Degree in any field with two years of experience working in a human service organization.
  1. 2-3 years of experience providing crisis intervention is preferred.
  2. A valid driver's license, reliable means of transportation, and proof of automobile insurance are required.

Salary Range: $47,840-$56,190 per year ($23.00 - $27.00 per hour) - dependent on education and experience
