WHAT YOU WILL BE DOING (ESSENTIAL DUTIES) ...

The Street Outreach Team Leader is responsible for overseeing a team of street outreach case managers dedicated to engaging with vulnerable populations, including individuals experiencing homelessness, substance abuse, or mental health challenges. This role involves direct supervision, program development, and community collaboration to ensure that individuals receive the necessary support and resources. Reports to Housing Director

Essential Duties:

  • Lead, mentor, and support outreach team members in their daily activities.
  • Conduct regular individual supervision, team meetings, performance evaluations, and training sessions.
  • Design and implement effective outreach strategies to engage target populations.
  • Analyze outreach outcomes and adjust strategies as needed to maximize impact and report program deliverables
  • Establish and maintain relationships with local organizations, service providers, and community stakeholders.
  • Represent the organization at community meetings, events, and coalitions.
  • Provide guidance and support to team members during crisis situations.
  • Ensure the safety and well-being of clients and staff during outreach activities.
  • Oversee data collection processes related to outreach efforts and client interactions.
  • Prepare reports and presentations on outreach activities and outcomes for management and stakeholders.
  • Identify and connect individuals with housing, healthcare, mental health services, and other support resources.
  • Stay informed about available community resources and best practices in outreach.

WHAT WE EXPECT OF YOU...

  • At least two year of program management or supervisory experience in a mental health setting or equivalent
  • Associates degree or higher.
  • Ability to multi-task and thrive within a fast-paced environment
  • Exceptional communication skills with the ability to professionally communicate verbally and in writing with a variety of individuals
  • A solid understanding of program management best practices and the ability to work effectively to meet deadlines and goals
  • A track record in setting priorities, shaping processes and developing infrastructure to ensure team productivity and efficiency
  • Demonstrated understanding and respect for the cultural and religious beliefs of clients and families
  • Valid Driver's License with reliable transportation to travel to our facilities throughout the area.
