Job Description Overview

Project Access is a community based organization with the mission of increasing access to care and services for residents of the greater New Haven Community. Under the supervision of the Executive Director of Project Access of New Haven (PA-NH), the Navigation Manager/Clinical Director serves in the dual role of Manager for the Project Access Uninsured Specialty Care Navigation Program and Clinical Leader for Project Access. This leadership role will provide oversight for the uninsured patient navigation program including staff recruitment, supervision, managing program goals and targets, staff training and volunteer physician recruitment, stewardship and retention. As the Clinical Director this role provides clinical oversight and consultation for patients enrolled in the program working collaboratively with referral sites and volunteer medical care providers.

Responsibilities

  • 1. Manages grant proposal development and reporting for the organization
    • 1.1 Utilizing clinical expertise, and set criteria, accept or decline referrals
  • 2. Responsible for the design and implementation of PA-NH research and program evaluation initiatives
    • 2.1 Ensure that appointments are made in a timely fashion
  • 3. Develops surveys and data collection instruments
    • 3.1 Oversees daily morning huddle with Patient Navigator staff to review workload for the day and discuss clinical and psychosocial needs of patients
  • 4. Reports program progress internally and externally to stakeholder groups
    • 4.1 Establishes relationships with other community service organizations, as well as pathways for referrals and collaboration
  • 5. Leads the PANH Medical Advisory Committee
    • 5.1 a. Coordinates the on-call provider schedule
  • 6. Attendance at conferences and meetings
    • 6.1 Represents PANH and YNHH at various committee meetings as necessary
  • 7. Recruitment and retention of staff
    • 7.1 Oversees the recruitment and hiring of program staff

Qualifications

EDUCATION

RN Required, B.S.N. M.S.N. or N.P with at least five years of experience in a Med./Surg. or Emergency Room setting

EXPERIENCE

Minimum 5 years clinical experience in a specialty care setting required with a working knowledge and expertise in understanding diagnosis, medications, therapies and discharge planning, as they relate to patient care

LICENSURE

Licensed Registered Nurse or advanced practice certification if applicable

SPECIAL SKILLS

Registered nurse, physicians assistant, nurse practitioner. Fluency in Spanish preferred (not required). Enjoys working collaboratively with professional and non - professional colleagues alike


Additional Information

1-3 years experience, preferably in cardiovascular nursing and specifically heart failure. This position supports the in-patient navigator as well as HF clinic alongside the APP and MD 1.5 days a week.
