ADRC Advanced Professional: Options Counseling - Youth Transition

The Aging and Disability Resource Center (ADRC) of Milwaukee County provides FREE information and assistance regarding community resources and services for adults. ADRC staff assist individuals to plan for short-term and long-term needs, access community services and resources, and learn about programs and benefits available. Promoting Independence and Inclusion.

Job Summary: The ADRC Advanced Professional is a single point of contact for ADRC customers needing a more streamlined, specialty approach to meet their needs and wanting to transition from youth to adult services in Long-Term Care. This position possesses advanced knowledge of ADRC services and works with customers from 'beginning to end' providing all services that are typically performed by multiple staff. This position also serves as a mentor to new hires, thoroughly showing them all aspects of their daily work for an extensive period during onboarding. This ADRC Advanced Professional exists to meet the needs of customers who have high acuity, increased need for assistance, and lack natural support.

Job Duties:



  • Upon receiving a specialty, high acuity, or expedited referral, contact customer to assess situation, needs, urgency of need, and preferences. Identify resources to meet customer needs based on customer demographics, goals, and resource eligibility. Collaborate with support persons (professional and non-professional) involved in the customer's life. Communicate complicated information in a manner understood by customers who often have cognitive or physical disabilities. Complete follow-up contacts to ensure customers' needs were met. Be knowledgeable of and make use of multiple systems to facilitate customer service: Forward Health, FSIA, CARES, online resource databases, MIDAS. These systems confirm eligibility for public benefits and contain extensive information on thousands of resources and customers in Milwaukee County. Provide decision-support to customers navigating the complex world of long-term care. Achieve and maintain Options Counseling Certification and AIRS Certification throughout duration of employment.
  • Achieve Long-Term Care Functional Screen (LTCFS) certification within 1 month of beginning employment. Maintain certification throughout duration of employment with satisfactory quality reviews and by attending ongoing trainings, maintaining knowledge of state-prescribed instructions and any updates to them, and passing the biennial recertification exam. Utilize motivational interviewing skills with customers and collateral contacts to complete functional assessments covering Activities of Daily Living, such as bathing, dressing, medication management, money management, cognition, memory, and risk. Score abilities according to state instructions and describe observations in professional, objective, detailed narrative in the Gateway/FSIA system. Review and research medical diagnoses and records as they relate to functional deficits. Per WI DHS LTCFS instructions (p-00946), "The screening process requires face-to-face contact with the individual being screened" and "telephone contact, video conference, internet video" are not considered face-to-face contact.
  • Work with customers and their legal decision-makers to complete Medicaid applications. Provide the level of assistance necessary for person-centered services when collecting verifications. This can include helping the customer contact their bank or life insurance policy holder to request documents and picking up verifications from customers' homes. Accurately respond to questions from customers about their financial eligibility for full-benefit Medicaid programs, including questions about the application process, cost share, patient liability, how their income/assets affect eligibility, and the specific verifications needed for an application. Requires a comprehensive knowledge of various types of Medicaid and youth services, including Community Waivers, Institutional, and Children, Youth, & Family Services (CYFS).
  • Accurately and thoroughly document each interaction with customers in the case management system with case notes that must meet state and local standards. Complete state-required encounter reports for every direct contact with customers and collateral contacts. Maintain 100%-time reports in 15-minute increments to receive financial reimbursement from the State of WI. Maintain confidentiality. Update customer records as needed with current demographics.
  • Be a mentor to newly hired Options Counselors by allowing them to shadow all daily work for 4 to 8 weeks. This is supplemental to supervisor's orientation and other required trainings. Demonstrate the work the new hires will be required to do, including Options Counseling, functional screens, and Enrollment Counseling per contractual and best practice guidelines. When ready, allow new hires to take the lead and shadow/support them. Provide feedback to help them master their skills and work independently upon mentorship conclusion. Serve as a lead point-person for other ADRC Professionals who have questions about processing cases.
  • Possess extensive knowledge of WI's Long-Term Care Medicaid programs (Family Care, Partnership, IRIS, and PACE), services, and eligibility. Help customers understand the programs without influencing their decision, per federal regulation. Provide unbiased information on four Managed Care Organizations and four IRIS Consultant Agencies for customers to choose which company in which to enroll. Assist IRIS customers in understanding and choosing one of four Fiscal Employer Agents. Explain to each individual customer how enrollment or disenrollment from Long-Term Care Medicaid programs could impact their specific situation, including financial and personal impact.
  • Support and complete pending cases of staff who leave the ADRC. Investigate to determine where case is in process of Options Counseling/LTC/CYFS. Effectively establish renewed rapport and trust with the customers. Finish processing case to closure.
  • Collaborate with colleagues from the various School Systems, Division of Vocational Rehabilitation, WI DHS, Managed Care Organizations, residential facilities, hospital systems, corrections, etc. regarding the transition of cases to Publicly Funded Long-Term Care. Maintain contact with liaisons from these agencies, track the progress of cases, and document all activity.
  • Perform other duties as needed.



Minimum Qualifications:



  • Current resident of the State of Wisconsin.
  • Valid Wisconsin Driver's License and reliable insured vehicle; able to be maintained throughout employment.
  • Bachelor's Degree from an accredited college or university in Human Services or in a closely related field.
  • At least Two (2) years of experience working in an Aging and Disability Resource Center (ADRC) or closely related field.
  • Intermediate experience with all related computer and software applications such as Microsoft Word and Excel.
  • Intermediate experience with FSIA, case management system, CARES, ForwardHealth systems
  • Routine shifts hours. Infrequent overtime, weekend, or shift rotation.
  • Intermittent or cyclical work pressures with occasional exposure to high stress work environments.



Note: Long-Term Care Functional Screen certification within one month of employment.

Knowledge, Skills, and Abilities: Skills needed for success in this role include customer focus, excellent verbal communication skills, patience, ability to write succinctly, professionalism, critical thinking, strong organizational skills, and time management skills. The ability to work collaboratively with other units and organizations within and throughout the county. Proficient in data collection and analysis.
