Comagine Health is looking for a Home and Community Based Service (HCBS) Assessor based in Northern New Mexico. This person because of the position will be required reside in the NW region of NM which includes San Juan, McKinley and Cibola counties. In this role, you will conduct in-home assessments for developmentally disabled participants electing to receive services through the New Mexico Mi Via Waiver. This important work includes administering face-to-face assessments, coordinating meeting schedules with participants (and their designated agents), and documenting assessment results into the care management system. What you'll be doing for us:



  • Meet in-person with eligible participants to assess qualification for the Mi Via developmentally disabled waiver program.
  • Utilize Vineland assessment tool/software to evaluate domains such as communication, daily living skills, social skills, motor skills and maladaptive behaviors.
  • Schedule and conduct assessments.
  • Coordinate assessment sessions with eligible participants and others involved.
  • Complete and document Vineland assessment results in Jiva.
  • Contribute to the orientation and training of other non-clinical employees.
  • Submit assessment results to Clinical Reviewers.
  • Provide input, suggestions, and recommendations per case to Clinical Reviewers.
  • Participate in quality management tasks.
  • Consult with colleagues and manager through regular meetings and more often as needed.



Competencies:



  • Superior interview skills necessary to obtain information about an individual's adaptive functioning.
  • Proven experience assessing the adaptive behavior of individuals with intellectual and developmental disabilities.
  • Ability to effectively communicate and interact with individuals with intellectual and developmental disabilities and their caregivers.
  • Excellent interpersonal, and oral and written communication skills.
  • Ability to make determinations regarding an individual's adaptive functioning and rate their adaptive functioning using software and other assessment tools.
  • Strong knowledge of Microsoft Office Suite.
  • Strong Knowledge or HIPAA, and Confidentiality practices.
  • Experience using electronic health record systems (EHR) or other clinical documentation systems.
  • Ability to enter accurate and complete data into the EHR.
  • Experience with Medicaid preferred.
  • Experience using Vineland assessment adaptive behavior assessment evaluation tool/software preferred.
  • Verifiable good driving record and reliable transportation.
  • Strong organizational skills.
  • Ability to work independently in a telework or field setting.
  • Ability to work within a team for support of clinical and non-clinical AMCCI program needs.



Required Qualifications:



  • AA/AS in social work, psychology, other behavioral health science field or nursing with relevant experience / training in behavioral health services.
  • 2+ years of social service, nursing or other relevant work experience.



Desired Qualifications:



  • Bachelors in social work, psychology, other behavioral health science field or nursing with relevant experience / training in behavioral health services.
  • Licensure in a mental health field.



Salary Range: $45,000 - $50,000

The salary range posted reflects the range that Comagine is willing to pay for this position. Salary is determined by many factors, including but not limited to geographic location of where the employee will perform their job duties in addition to their knowledge, skills, education, and relevant work experience.
