POSITION SUMMARY

We are seeking a dedicated and compassionate Licensed Clinical Social Worker for our Ambulatory Clinic - Endocrine and Weight Wellness team. The ideal candidate will play a crucial role in ensuring patients have access to the resources and care they need to manage their diverse endocrine needs.

* Shift: 8-hour days, Monday through Friday

* Hybrid role. You will begin working each day in the office with the eventual ability to work from home a few days per week with mutual consent from leadership.

EXPERIENCE AND EDUCATION

REQUIRED

* Master's degree in Social Work

* And three (3) years' experience in medical and/or psychiatric social case work to include one (1) year as Social Worker II.

* LMSW (Licensed Master Social Worker) required.

PREFERRED

* LCSW (Licensed Clinical Social Worker) may be required in some departments.

JOB DUTIES

* Patient Advocacy: Act as a liaison between patients and healthcare providers to ensure patients' needs and concerns are addressed.

* Resource Coordination: Identify and connect patients with community resources, financial assistance programs, and support services to enhance their access to care.

* Content Expert: Developing comprehensive education and streamlined workflows to ensure clinicians and staff are well-informed about the various options available for patients.

* Care Planning: Collaborate with the healthcare team to develop comprehensive care plans that address the medical, emotional, and social needs of patients.

* Education and Counseling: Conduct comprehensive assessments of patients to understand their needs and challenges. Provide education and counseling to patients and their families about managing endocrine disorders, treatment options, and lifestyle changes.

* Provide psychotherapy and supportive counseling using current DSM and evidence-based guidelines.

* Actively participate in clinic sessions alongside interdisciplinary teams. Crisis Intervention: Offer support and intervention during times of crisis, helping patients navigate complex healthcare systems and overcome barriers to care.

* Follow-Up and Monitoring: Conduct regular follow-ups with patients to monitor their progress, address any ongoing needs, and adjust care plans as necessary.
