Full-time Discharge Planner Opening The Pavilion Behavioral Health System has been the leading provider of behavioral health and addictions treatment for families in Illinois since 1989. Visit us online at: www.pavilionhospital.com. The Discharge Planner must be able to demonstrate competency in the criteria listed in this job description. It is imperative for the Clinical Case Coordinator to have outstanding organizational skills and the ability to constantly multi-task. Primary responsibilities include the coordination of clinical staffings, making collateral contacts and arranging discharge plans/linkage with aftercare services. Additional responsibilities include the provision of clinical services to psychiatric inpatient and partial hospitalization (PHP) patients and their families. Benefit Highlights:

Previous professional experience in a behavioral health setting preferred.
