This position has the opportunity to be housed within our St. Cloud or Brainerd office, and will work across the central Minnesota region, working with youth under the age of 25 who have experienced sexual exploitation or trafficking. Job Summary: As a Case Manager you will provide case management services to individuals and youth in transitional living and/or within the community. You will manage the client intake/discharge process, work with program participants to develop case plans/goals and monitor guide and assist them to carry out case plan tasks. You will meet with program participants on a regular basis and communicate often with family members, professionals and outside agencies involved in client transition regarding client progress. You will complete assessments, determine eligibility for government and community services, make referrals and develop a comprehensive follow up system to maintain a contiguous path of services. In this role you will facilitate weekly client groups and/or client recreation activities, build new relationships with community organizations and employers, build and maintain collaborative relationships with a range of providers for current and future services and promote/represent the program at community gatherings. You will maintain accurate and current case files, program records, reports and other necessary documentation. Minimum Requirements:
