Compensation Grade: P18Compensation Details: Minimum: $63,887.00 - Maximum: $63,887.00 AnnuallyDepartment (OPH) CCH - Division of Chronic Disease PreventionJob Description: Responsibilities The Health Program Coordinator will contribute to programmatic activities for the New York State Heart Disease and Stroke Prevention Program (HDSPP). This position will support work for three grants funded by the Center for Disease Control and Prevention (CDC), including the National and Innovative Cardiovascular Health Programs and the Coverdell Stroke Program. The incumbent will work as part of a team to achieve work plan deliverables across all three programs. This work includes contributing to contract management activities, coordination of meetings and a quality improvement learning collaborative and communicating with internal and external partners. Minimum Qualifications Bachelor's degree in a related field and two years of experience contributing to the coordination of program activities in a public health, health, health regulatory, or human services related program; OR an Associate's degree in a related field and four years of such experience; OR six years of such experience. A Master's degree in a related field may substitute for one year of experience. Preferred Qualifications Experience working with health systems and/or community-based organizations; experience developing and executing contracts/vouchers; experience coordinating or providing organization to a large scale project; experience with heart disease and stroke initiatives; experience with quality improvement initiatives; experience with applications including Word, Excel, PowerPoint, teams, zoom/Webex. Conditions of Employment Grant funded position. Compliance with funding requirements such as time and effort reporting, grant deliverables, and contract deliverables, is required. Valid and unrestricted authorization to work in the U.S. is required. Visa sponsorship is not available for this position. This position requires that the incumbent will report to the official work location and live within commuting distance to the official work location. Telecommuting will be available.
