This position is eligible for a sign-on bonus! The YCCS Program (Youth Community Crisis Stabilization) Program based in Walpole, MA is seeking a Licensed Practical Nurse. Under the direction of the YCCS Nurse Manager, the Licensed Practical Nurse will administer medication, provide nursing care, manage the ongoing health care of the youth and provide leadership on the shift in the YCCS. The Licensed Practical Nurse is expected to be knowledgeable of the agency's mission, program treatment philosophy, policies, and procedures and to implement this understanding in their care of the youth. How You'll Be Making A Difference
- Maintain a comprehensive understanding of the population of youth in need of short-term mental health stabilization.
- Assist in the development of an individualized health services plan using a collaborative, wellness focused approach. Participate in the implementation of the plan and monitor effectiveness.
- Assist with filling physician's orders, administering medications and monitoring vital signs.
- Prepare leave of absence medications and or transfer/discharge medications.
- Provide leadership on the shift by supporting the implementation of the daily routines to ensure that the environment promotes safety, recovery and treatment. Provide direction to Mental Health Technicians on how to engage the youth in programming.
- Assist in making decisions regarding the course of treatment and monitoring the effectiveness, side effects, adverse reactions, and drug interactions of all medications.
- Assist with implementing comprehensive health/wellness programming, including environmental, nutritional, exercise and fitness in collaboration with a multidisciplinary team.
- Complete reports and documentation needed for youth records, for the Home, collaterals and regulatory agencies.
- Provide health education for youth and families and assist with training for staff.
- Assist in the accurate receipt of medication and medical supplies.
- Conduct health screenings upon intake and as necessary and assure that all immunizations are current.
- Assist with medical, diagnostic and laboratory appointments and initiate appropriate action.
- Contribute to the nursing assessment as part of the comprehensive assessment process.
- Provide ongoing assessment of youth including vital signs as prescribed.
- Provide brief crisis counseling as needed and assist with crisis prevention planning.
- Assure compliance with all regulatory guidelines.
- Assist in the delivery of health care with ancillary staff, family members, guardians, psychiatrists, primary and specialty health care providers, and other professionals within the Home and with external organizations. Provide on-going communications with primary health care provider.
- Work actively as part of an interdisciplinary team to provide quality care, attend team meetings and communicate regularly with team members. Assist with discharge planning.
Qualifications
- Two years pediatric/adolescent and or psychiatric nursing experience preferred.
- Must maintain an active LPN license in Massachusetts.
- Certified in CPR and Standard First Aid
- Computer literate including Microsoft Office and Internet Explorer with the ability to learn new software applications.
