Human Services Case Manager Case Management Support Specialist will provide refugee resettlement and integration support for ongoing and collaborative efforts by federal, state, and local governments, NGOs, non-profits, resettlement agencies, and community organizations. Compensation & Benefits:
- Estimated Starting Salary Range for Human Services Case Manager: $ 34.00 - $36.00 hourly
- Pay commensurate with experience.

Human Services Case Manager Responsibilities Include:
- Responsible for client intake, assessment, client documentation and record maintenance for all individual refugee cases assigned.
- Responsible for screening refugee clients for eligible service to include but not limited to Refugee Cash Assistance (RCA), Refugee Medical Assistance (RMA), Social Adjustment Services (SAS) to include employment services and referral, English as Second Language (ESL) and all other applicable programs and record all documentation in statewide database.
- Collect and upload client eligibility documents.
- Monitor client compliance with program eligibility and requirements.
- Maintain all required documentation for client benefits and issue payment requests for monthly allowances.
- Monitor and evaluate client progress.
- Maintain and complete all required documentation to ensure compliance with funding requirements and agency quality assurance standards.
- Prepare and maintain detailed case files, required forms, reports and Case notes.
- Enter all required information into statewide databases and agency databases.
- Advocate for clients with service providers or agencies in the community.
- Collaborate with other agencies and community partners or resources to meet the needs of the client.
- Maintain HIPAA and privacy of all client information.
- Collaborate with local health departments, social services offices, mental health providers, school systems and other services providers to establish referrals when appropriate.
- Ensures language and cultural competency, mental health and social services in support of program.
- Performs other job-related duties as assigned.
Human Services Case Manager Experience, Education, Skills, Abilities requested:
- Bachelor's Degree in social work, counseling, or related fields preferred
- at least 2 years of professional experience in case management services or related field, or an equivalent combination of education and experience.
- Bilingual communication skills are required.
- Ability to communicate clearly and effectively via oral and written means.
- Ability to present a positive demeanor to individuals, their families, staff, supportive services personnel, and the general public.
- Ability to remain calm during difficult circumstances and emergencies.
- Able to effectively manage significant documentation and data.
- Efficient knowledge in utilizing Outlook, Word, and Excel; PowerPoint, Access, Visio.
- Must pass pre-employment requirements of Cherokee Federal.
