Advance Care Planning Nurse

Santa Barbara Cottage Hospital seeks a full-time Advance Care Planning Nurse to support the Advance Care Planning Program with a focus on our Spanish speaking population. In this role you will provide advance care planning (ACP) facilitation to patients and families, process advance care documents received from community members, develop and present education to hospital staff and medical providers about ACP, advance care documents and Physician Orders for Life Sustaining Treatment (POLST), and attend community meetings regarding advance care planning.

Qualifications
* California RN license
* BSN
* 1+ years' serious illness or hospice care experience, including end of life discussions with patients and family members, and advance care planning training.
* Fluent in Spanish with ability to obtain interpreter certification within one year of assuming the role.
* ELNEC certification and POLST training preferred
