Presbyterian Healthcare Services is seeking a dedicated and compassionate Certified Diabetes Educator (CDE) to join our team. This full-time position, that is remote eligible, offers the opportunity to make a significant impact on the lives of individuals with diabetes. The CDE will work closely with physicians and Advanced Practice Clinicians (APCs) to provide comprehensive diabetes management and education, ensuring continuity of care between visits.

Key Responsibilities:

    Patient Consultation: Conduct initial consultations with new patients lasting 60 minutes and follow-up sessions for 30 minutes, averaging 13-14 patients per 8-hour day.
  • Collaborative Care Management: Assist physicians or APCs in managing patients' diabetes care between scheduled visits.
  • Treatment Adjustments: Authorized to make necessary insulin and pump adjustments to optimize patient care.
  • Team Integration: Collaborate with physicians/APCs to deliver patient-centered care.
  • Education and Training: Provide pump education, pre-teaching, carbohydrate counting, and ensure patients sign the pump contract.
  • Protocol Development: Develop and implement clinic protocols to streamline patient care processes.

Qualifications

Experience:

3-5 years experience in nutrition

Education:
Bachelor Degree in Nutrition and Food Science

Credentials:

Registered Dietitian
Certified Diabetes Educator

BLS required at time of hire.

Preference:

1 year experience with Insulin Pumps and Continuous Glucose Monitoring (CGM)

3-5 yrs experience in a community or clinic setting, and diabetes education experience
