Position Summary This position requires full understanding and active participation in fulfilling the mission of Saddleback Medical Center. The employee shall support Saddleback Medical Center's strategic plan and participate in and advocate performance improvement/patient safety activities. The RN Nurse Navigator's primary function is assisting patients and families through the healthcare system with the goal of achieving optimal patient outcomes. The RN Nurse Navigator will provide cancer patients and families with information addressing their concerns, including but not limited to financial, social services, education, treatment options and access to clinical trials. The RN Nurse Navigator may serve as a care coordinator for multi-disciplinary teams. The RN Nurse Navigator is a self-directed individual who is an expert practitioner with essential educator and case manager role components. The RN Nurse Navigator works closely with the clinical staff, physicians, Tumor Boards and community resources. Essential Functions and Responsibilities of the Job:
