Pharmacist - Infusion Therapy

The NCH Homecare Pharmacist is responsible for dispensing patient medications; maintaining medication profiles; developing Plans of Care on each homecare patient, when required; documenting therapy interventions; providing drug information; coordinating care planning with NCH nursing staff and referral sources; assisting in supervising technician/delivery personnel; coordinating delivery schedules for home patient therapy, and performing miscellaneous duties as requested. Demonstrates competencies defined (*) in NCH Homecare Pharmacist responsibilities in providing age-appropriate care to patients from the age of birth through 21 years, and select adult patients.

Responsibilities

    Dispenses medications for NCHH patients; receives orders, calculates, and oversees compounding of I.V. admixtures and total parental nutritional solutions; manages dispensing information via computer and prospective drug utilization review for appropriateness.
  • Coordinates and collaborates with patients, their families, nursing providers, discharge planners and/ or prescribers in the development of list of needed supplies and equipment, including infusion pumps and devices for medication administration as well as catheter care.
  • Develops and maintains a medication list and an individualized pharmacy plan of care that documents desired outcomes and potential problems using communications with patients, families, and prescribers.
  • Documents patient care, clinical, operational, and financial outcomes and activities appropriately in the medical record.
  • Provides in-service training to medical, nursing, and pharmacy staff. Precepts and/or supervises pharmacy residents, students, interns, technicians, or graduates awaiting licensure. Provides appropriate oversight and input in performance evaluations.
  • Actively participates in NCHH continuous quality assurance and performance improvement activities (including competencies for USP 797 and 800 processes and other oversight regulations, zero hero tracking and reporting, patient case conferences, performance improvement teams, customer satisfaction surveys, utilization review activities, etc.)
  • Shares in on call responsibilities


Qualifications

KNOWLEDGE, SKILLS AND ABILITIES REQUIRED

Education:

  • Doctor of Pharmacy (preferred) or Bachelor's Degree from an accredited college of pharmacy.

Licensure:

  • Licensure, or licensure-eligibility, to practice pharmacy in the state of Ohio.

Certifications:

  • Possible Board Certification Sterile Compounding Pharmacy.

Experience:

  • Homecare, Home Infusion or Hospital experience or internship desirable.
