UT Southwestern Medical Center has a new opportunity available for the role of Social Worker - Home Health. This is a PRN position and the ideal candidate will provide coordinated care in the home to clients and assists with creating patient centered action plans to promote effective education, self-management support, and adherence to the PCP provider's plan of care facilitating access and timely healthcare delivery on an added basis. You will see patients who require skilled care after an in-patient hospital visit and determined what additional services are required such as meals on wheels, care plans, etc. Services typically last 9 weeks once assigned. The ultimate goal is to assist the patient to independence within the stated time frame.


EXPERIENCE and EDUCATION:

  • Masters degree in Social Work with at least one (1) year of experience in a health care setting preferable with homebound patients.
  • Current license is required by the Texas State Board of Social Work Examiners.
  • Temporary license is acceptable during the first 90 days of employment.


JOB DUTIES:

  • Ensures the delivery of quality social service for patients by identifying patient needs and developing social service programs to meet those needs.
  • Participates in development and update of Plan of Treatment. Evaluates patient and home environment to determine social and psychological factors related to patient's health problems.
  • Prepares and submits to agency psychosocial histories, clinical and progress notes.
  • Observes and reports on patient/family reaction to treatment.
  • Counsels patient and/or family in understanding, accepting and adjusting to patient's health problems and emotional care.
  • Identifies, informs and refers patient/family to available community resources.
  • Coordinates discharge planning. Interprets to the community the role of social work in the Agency's programs.
  • Participates in planning, development and delivery of staff in-service education programs.
  • Participates in Continuing Quality Improvement Program.
  • Monitors adherence to policies, procedures and protocols.
  • Performs other related duties as assigned or requested.


KNOWLEDGE, SKILLS & ABILITIES:

  • Work requires good general health and stress coping ability.
  • Work requires full mobility and ability to participate in vigorous activity.
  • Work requires ability to speak and hear, to allow discussions with physicians, patients, and other members of the healthcare team.
  • Possess near vision for accurate reading of computer screens and recording on patient charts.
  • Must be able to walk reasonable distances to cover multiple nursing units.
