About the Department This Serious Illness Care Navigator will be focused on people with advanced heart failure navigating complex health journeys. Work of scope The Serious Illness Care Navigator (SICN) will assess the patient's individual needs and develop a plan of care to meet those needs. This role includes providing education and support to patients, families, staff and caregivers.
