We are pleased to present the following position for your consideration: Overview: Family Resource Developer This full-time position will join the Heritage Youth and Family Support Services team to provide case management and community support services to parents and caregivers of youth receiving services at Heritage. Outreach services will be provided in array of settings including an individual's home, community settings, community agencies, schools, as well as on-site at Heritage. A successful candidate must have lived experience navigating community systems for their child or personal mental health or substance use recovery. Core Responsibilities Include:
- Identifying family needs (i.e. access to health care, safety, and finances) and linkage to appropriate community resources
- Facilitate the development of supportive family relationships
- Development of natural supports
- Navigation of systems the youth/family are involved in
- Providing the family with intervention strategies and support.
Knowledge, Skills and Abilities:
- Ability to work with individuals with a mental health and/or substance use diagnosis;
- Ability to meet people where they are at in their recovery, non-judgmental, and ability to build rapport quickly;
- Ability to work as a team player;
- Ability to accept ambiguity, uncertainty, and change;
- Ability to navigate electronic health records;
- Ability to respond to supervisors, person served, and other clinical staff providing services to the person served.
- Candidates should be energetic and internally driven to assist youth to succeed in obtaining their desired outcomes
- Organized and able to develop a program plan with goals, objectives, and timeframes that are consistently met
- Knowledge of community resources and/or how to find
- Previous experience working with youth is highly recommended
- Effective oral and written communication skills
- Strong customer service skills
- Proficient in Microsoft Word and Excel
Education and Experience:
- High school diploma or equivalent is required; Bachelors degree in a relevant human service field (or 5 years of experience in the social service field and ability to function as mental health professional is highly recommended.
- Must be a minimum 21 years of age.
- Candidates will be expected to complete certification as a Peer Support Recovery Specialist or Family Partnership Professional within two years of employment.
- Valid driver's license, reliable means of transportation, and proof of automobile insurance are required
Salary Range: $47,840-$56,160 per year (dependent on education and experience) Heritage also offers the following with this position:
