Diabetes Educator RN

Good Shepherd Hospital

Inpatient and Outpatient

Part Time Days (32 hours per week)

Major Responsibilities:

  • Responsible for the coordination of education of patients, significant others and community related to diabetes self management as part of the continuum of care.
    • 1)Conduct and document individualized assessment of pertinent medical, psychosocial, and educational history of patients.
    • 2)Develop, implement, and document teaching plan, educational interventions, and follow-up plan based on the learning needs assessment for patients, families, and/or significant others.
    • 3)Collaborate with patients, families, and/or significant others in establishing mutual learning goals and behavior change goals.
    • 4)Document evaluation of outcomes, and revises interventions and follow-up plan for patients, families, and/or significant others.
    • 5)Access resources to facilitate discharge planning and appropriate follow-up.
    • 6)Assure education is based on current standards of practice and input from physicians.
    • 7)Participate in planning and conducting community service projects and programs.
    • 8)Communicate education and follow-up recommendations to physicians and other health care providers within the multidisciplinary team
    • 9)Implement and coordinate outpatient education and follow-up needs.
    • 10)Bill third party payers for diabetes outpatient self-management and training, utilizing appropriate coding guidelines
  • Functions as a resource in diabetes management for the healthcare team.
    • 1)Serves as a resource person to the healthcare team regarding procedures, clinical pathways, and equipment directly related to diabetes management.
    • 2)Acts as a resource to other departments and disciplines within hospital and community.
    • 3)Participates in formal and informal educational programs regarding diabetes-related topics.
    • 4)Maintains knowledge of current developments, trends, and techniques in diabetes management and education and makes recommendations regarding use in diabetes program.
    • 5)Coordinates the standardization of patient education materials for education in patient care departments.
    • 6)Identifies educational needs of the health care team related to diabetes and provides opportunities for learning.
  • Incorporate activities of quality improvement to ensure positive patient outcomes and experience.
    • 1)Participates in formulating, implementing, and evaluating continuous quality improvement activities for the diabetes education program and materials.
    • 2)Participates in professional organizations related to the subspecialty in order to promote and maintain current standards of care and patient education.
    • 3)Participates in diabetes advisory committee and others as appropriate.
    • 4)Integrates research findings into nursing care and education of patients with diabetes.
    • 5) Meet with sales representatives regarding products and supplies for patient education.

Licensure:

  • Nurse, Registered (RN)

Education/Experience Required:

  • Bachelor of Science in Nursing. 3-5 years experience in clinical specialty. .

Knowledge, Skills & Abilities Required:

  • Strong leadership skills. Ability to communicate and work cohesively with diverse groups. Excellent organizational and planning skills.
  • Specialty certification in Diabetes (CDE) preferred or must be attained within 18 months of hire. Current license in the State of Illinois as a registered nurse. CPR

Physical Requirements and Working Conditions:

  • Ability to travel locally. Ability to function in a fast paced environment with established time constraints. Ability to handle multiple tasks and complete work within a short time frame. Resilient and flexible in a changing environment. Ability to cooperate and work well with others. Ability to make sound and timely decisions. Ability to flex hours in order to accommodate needs of patients.
  • If position has direct patient care or direct patient contact the following lifting requirement supersedes any previous lifting requirement effective 06/01/2015. Ability to lift up to 35 pounds without assistance. For patient lifts of over 35 pounds, or when patient is unable to assist with the lift, patient handling equipment is expected to be used, with at least one other associate, when available. Unique patient lifting/movement situations will be assessed on a case-by-case basis.
