This position is Part Time, 30 hours. The Clinical Research Nurse is an integral member of the Pulmonary Transitions Program team at the OSU East Hospital. The main goal of this program is to provide dedicated transitional care for hospitalized pulmonary patients to ensure safe and effective discharge, as well as facilitating timely access to pulmonary outpatient care. The Clinical Research Nurse will identify suitable patients to benefit from this program and facilitate transitional care starting just prior to hospital discharge. This will include patient education, multi-disciplinary collaboration with the inpatient team for discharge planning and functioning as a liaison with the outpatient Pulmonary Transitions Clinic team. The Clinical Research Nurse has a central role in identifying suitable patients who would benefit from this program, specifically patients admitted for primarily pulmonary-related problems, including exacerbations of chronic obstructive pulmonary disease or asthma, interstitial lung disease, acute cough, shortness of breath, respiratory failure, pleural effusions, infectious pneumonias and pneumonitis. Additionally, this position will involve providing bedside patient education, which includes smoking cessation counseling, inhaler teaching, and explaining medical concepts related to the patients disease process with the patient and/or family. This role requires close collaboration with a multi-disciplinary inpatient team to minimize barriers to successful discharge. For example, this may involve collaborating with social work to address transportation barriers and/or the pharmacist to identify and navigate prohibitive costs of pulmonary inhalers. While this is an inpatient-based role, the Clinical Research Nurse will also be responsible for follow-up phone-calls to discharged patients within 48 hours of discharge to identify early problems post-discharge, with the support of an advanced practice provider (APP) and MD. Furthermore, the Clinical Research Nurse will function as a first point-of-contact for issues that arise for discharged patients prior to establishing with the outpatient Pulmonary Transitions Clinic. |
