Compensation: Pay rate will be determined based on level of experience. Responsibilities Position reports to the Manager, Community Benefits, or designee, in collaboration with a multidisciplinary care team build, leverage, and expand organizational support for equitable and accessible disease prevention and control among populations that are marginalized and disproportionately affected. Provide culturally appropriate health education, and outreach in a variety of settings including community-based, ambulatory, and acute care settings as needed. Implement culturally tailored interventions to meet patients short-term and long-term medical and associated health needs. Assist patients to practice healthy behaviours and access necessary social and medical support services and programs. Embed a health equity lens in all Community Wellness Program related activities.
