JOB SUMMARY
Under limited supervision, works in partnership with the OPAT team to allow patients to complete antibiotic treatment safely and effectively in the comfort of their home or outpatient site. Coordinates care for infectious diseases patients, with specific focus on management of patients discharged with outpatient parenteral antimicrobial therapy (OPAT) services for follow up in the Infectious Diseases (ID) Clinic. Major responsibilities include care coordination with inpatient providers for patients being discharged on OPAT, and ongoing monitoring of OPAT patients after discharge. The nurse will work as a member of a multidisciplinary team to optimize patient outcomes associated with the use of antimicrobial therapy in the outpatient setting.

EXPERIENCE AND EDUCATION
Required



  • Education
    Graduate of an accredited Nursing program and


  • Experience
    3 years 3-5 of clinical experience as a Registered Nurse and


  • Licenses and Certifications
    (NLC) NURSING LICENSURE COMPACT-BNE Current Texas (or BNE Compact State) license; and
    (BLS) BASIC LIFE SUPPORT certification, course accredited by the American Heart Association (AHA) or American Red Cross (ARC)

JOB DUTIES




    • Performs job responsibilities in accordance with organizational and departmental policies and procedures, performance improvement objectives and environmental/infection control standards. Develops/implements process to meet/comply with the Joint Commission and regulatory requirements.
    • Assists clinical staff pharmacists with therapeutic monitoring, patient assessments, interpretation of and lab values. Identify opportunities for process improvements, develops and implements policy and procedures for prompt, safe and efficient patient care.
    • Coordinates and participates in multidisciplinary in-services/meetings,
    • Consistently works and problem solves independently and effectively. Takes charge when necessary and is looked to for direction. Makes the right decisions and uses good judgment. Mentors clinical staff pharmacists and supportive personnel.
    • Delegates appropriate/reasonable tasks and evaluates performance.
    • Reviews/updates medication profiles (therapies, allergies, height, weight, etc.) and is cognizant of patient charges. Reviews medication orders for therapeutic appropriateness, duplications, interactions, overall effectiveness and safety. Seeks clarification if necessary and documents interventions (i.e. ADRs).
    • Actively enhances personal growth and development by participating in educational programs, literature searches, in-services and meetings. Actively participates in new employee orientation and training and evaluations.
    • Duties performed may include one or more of the following core functions:

      • a. Directly interacting with or caring for patients;
      • b. Regularly maintaining, modifying, releasing or similarly affecting patient records (including patient financial records)


    • Takes ownership in solving problems effectively, efficiently and to the satisfaction of the patient
    • Presents constructive feedback regarding system or process failures and participates in resolution in a proactive and positive manner.
    • Maintains patient confidentiality in accordance with University Medical Center policy.
    • Consistently maintains control of feelings, emotions and body language.
    • Demonstrates through verbal and non-verbal communications openness to change or new ideas. Willingly learns and adapts to changes in work environment and new work methods as introduced. Demonstrates flexibility to fluctuating workload and effectively prioritizes assignments.
    • Exhibits a positive attitude in support of the hospital and management, including decisions made by administration and/or management. Treats hospital resources and property with respect and in a manner that reflects awareness of financial responsibility.
    • Adheres to all policies and procedures related to safety, infection control, and corporate compliance. Including personal demeanor, appearance, and hygiene that are appropriate for the work environment.
    • Responds to all reasonable requests in an appropriate and timely manner and communicates in a concise and positive manner.

    • Supports teamwork and cooperation with co-workers in daily activities. Readily volunteers assistance to help co-worker's complete tasks.
