Be the Key to Stability! we are seeking a compassionate and dedicated Housing Specialist to join our ACT team in Georgetown, DE and make a real difference in the lives of individuals with Serious and Persistent Mental Illness (SPMI). In this fulfilling role, you'll play a critical role in supporting individuals on their journey to securing and maintaining safe, affordable housing, fostering independence, and promoting overall well-being.

What You'll Do:
- Empower Housing Success: Guide clients through the housing search process, assisting them in identifying suitable options, completing applications, and navigating the leasing process.
- Develop Essential Skills: Equip individuals with the skills needed to thrive in independent living, including budgeting, conflict resolution, household management, and maintaining a safe and healthy living environment.
- Build Supportive Networks: Connect clients with essential community resources, including housing authorities, mental health services, and support groups, fostering a network that promotes stability and well-being.
- Champion Recovery: Integrate evidence-based practices and a trauma-informed approach to support clients in achieving their housing goals and fostering long-term recovery.
- Advocate & Collaborate: Develop strong partnerships with landlords, housing authorities, and community agencies to expand access to affordable housing options and advocate for the needs of your clients.
- Be a Pillar of Support: Provide ongoing support to clients as they transition into new housing, addressing challenges and ensuring successful tenancy.
- Champion Team Success: Collaborate effectively with a multidisciplinary team, sharing knowledge, and contributing to a positive and supportive work environment.
- Maintain Accurate Records: Document client interactions and progress meticulously, ensuring clear and concise case notes that adhere to all regulatory standards.
You're the Ideal Candidate if You Possess:
- A Bachelor's degree in Social Work, Psychology, Rehabilitative Counseling, or a closely related field.
- A minimum of one year of experience working as a Case Manager supporting individuals with SPMI or other mental health conditions.
- One year of experience in a housing support setting preferred.
- Strong communication and interpersonal skills, with the ability to build rapport and trust with clients from diverse backgrounds.
- Excellent organizational and time management skills, with the ability to prioritize tasks and meet deadlines.
- A commitment to evidence-based practices and a trauma-informed approach.
- A valid driver's license and reliable transportation.
