Terros Health is pleased to share an exciting and challenging opportunity for a Children's Wellness Coach, based out of locations in Phoenix, Avondale, Glendale, and Tempe, Arizona. This is a direct service position working in collaboration with the Boys and Girls Clubs of the Valley at assigned sites. This position will interact directly with the Children and Staff and Families at the Clubs and use the designated prevention model Positive Action while also implementing other clinical approaches based on Club needs. This team member needs to have a passion for working with Children in an afterschool setting, be willing to be an active participant/lead in therapeutic activities, and act as a liaison for Terros Health and these co-locations. This position will interact directly with the Children and Staff and Families at the Clubs and use the designated prevention model Positive Action while also implementing other clinical approaches based on Club needs. Full-Time Employed: 40 hours/week M-F: 11a-7p Starting at $19/hr Location: co-located within Boys and Girls Club of the Valley locations! - Conducts the engagement process with families to identify natural/formal supports as outlined in the Arizona Model, Child and Family Team (CFT) process, and Regional Behavioral Health Authority (RBHA) for enrolled families.
- Facilitates CFT/Service Plan meetings according to the Arizona Model and RBHA guidelines in order to create individualized service plans as identified by the CFT members.
- Ensures families understand the behavioral health process, the role of CFT Facilitator, and ensure family participation that links them to natural or community-based supports.
- Coordinates and schedules CFT/Service Plan meetings with natural, formal supports, and system stakeholders
- Submits appropriate referrals to clinical outpatient services within agency and other behavioral health providers identified by the CFT.
- Updates and maintains electronic medical records on each case which include but not limited to; Annual Assessment Update, RBHA demographics, Child and adolescent service intensity instrument (CALOCUS), and CFT/Service Plan
- Completes strength assessments and crisis/safety plans with families according to RBHA and the Arizona Department of Health Services (ADHS) guidelines.
- Develops positive supportive relationships with consumers, family, stakeholders, and other Terros employees and Boys and Girls Club employees. Actively leads group prevention or therapeutic group activities at assigned Boys and Girls Clubs of the Valley locations.

Qualifications
- Education/Experience:
- GED/High school diploma with a minimum of 1.5 years of behavioral health experience or,
- An associate's degree in the field of medical or behavioral health, or
- A bachelor's degree (not in the field of behavioral health) and 1 year of behavioral health experience, or
- A bachelor's degree in the field of behavioral, or
- A master's degree or higher in the field of behavioral health
- Preferred - Bilingual proficiency in English and Spanish.
- Computer Skills: Microsoft Word, Excel, Outlook, electronic medical record, NextGen preferred
- Valid AZ Driver License. Must have valid Arizona driver's license, be 21 years of age with minimum 4 years driving experience, and meet requirements of Terros Health's driving policy
- Must have a valid Arizona Fingerprint Clearance card (Level 1) or apply for an Arizona Fingerprint Clearance card within 7 working days of employment
- Interpersonal skills which allow for professional and positive relationships with co-workers, members, families, and other community members
- Good communication skills, evidenced by a non-judgmental approach, effective listening and positive interactions
- Basic understanding of the DSM-V preferred
- Certified Case Manager strongly preferred
- Must have or be able to attain CPR/First Aid certification within 60 days of assuming role
- Must pass background check, TB test and other pre-employment screening
