Position Summary The Crisis Care Coordinator is responsible for providing follow up services for individuals seen in crisis and helping coordinate referrals and warm hand offs to appropriate services in the community to help stabilize and/or mitigate future crisis The incumbent provides direct assistance in gaining access to services; coordinates care within and across systems; ensures linkage to appropriate services; conducts assessment of the eligible recipient to determine service needs; evaluates and completes a risk assessment; collaborates with law enforcement and other first responders to educate; works closely with all member of the crisis team and co-responders to help divert individuals from incarceration or unnecessary visits to the emergency department.Requirements

Pay and Benefit Expectations We provide a comprehensive benefits package, including competitive salary, medical, dental and retirement benefits and paid time off. As required by various pay transparency laws, we share a competitive range of compensation for candidates hired into each position. The pay scale is $27.86 - $40.08 USD. However, pay is influenced by factors specific to applicants, including but not limited to: skill set, level of experience, and certification(s) and/or education.
