Job Profile Summary

This role focuses on performing health assessments, running diagnostic tests, prescribing medication and creating treatment plans as well as providing health and wellness education to patients. In addition, this role focuses on performing the following Advanced Practice related duties: Under the direction of a physician, provides health care services to patients, ensuring optimal patient care. A management role that supervises employees focusing on tactical, operational activities within a specified area, with the majority of time spent overseeing area of responsibility, planning, prioritizing and/or directing the responsibilities of employees. Goal achievement is typically accomplished through performance of direct and/or indirect reports. A role that supervises all levels of employees. Responsibilities that typically include: Setting goals and objectives for team members for achievement of operational results, problems faced may be difficult but typically are not complex, and ensures policies, practices and procedures are understood and followed by direct reports, customers and stakeholders.

Job Overview

This position functions as a key member of the inpatient diabetes management team and provides specialized care for a case load of patients with endocrine disorders with a focus on all aspects of diabetes mellitus management. Practices in collaboration with the attending physician and other members of the multidisciplinary team to insure optimal patient outcomes. Provides patient and family centered care, providing psycho-emotional support, educational support and discharge planning that is individualized and comprehensive. Additionally, this position provides clinical consultation and participates in medical and nursing research, participates in continuing education, quality improvement initiatives, clinical outcome evaluation and monitoring.

Job Description

Minimum Qualifications:

1. Master's degree in Nursing.

2. Advanced Practice Registered Nurse (APRN) license.

3. Basic Life Support (BLS) certification.

4. Three (3) years of nursing experience.

Preferred Qualifications:

1. Certification as a Diabetes Educator (CDE)

2. Five (5) years of nursing experience.

Duties and Responsibilities: The duties and responsibilities listed below are intended to describe the general nature of work and are not intended to be an all-inclusive list. Other duties and responsibilities may be assigned.

1. Works with patients diagnosed with diabetes and their families to facilitate improving self-management of the disease with age specific goals, provide lifestyle coaching and work with patients on dietary education and modifications.

2. Performs procedures and treatments using advanced practitioner judgment and according to established practice guidelines and professional standards.

3. Demonstrates advanced knowledge of pharmacology/medications in ordering and administering drugs and monitoring the patient's response. Medication orders reflect this knowledge.

4. Remains knowledgeable about equipment and utilize equipment in a safe manner. These include, but are not limited to, continuous glucose monitoring (CGM) systems, insulin pump devices and related equipment, blood glucose meters and insulin administration devices.

5. Writes clear, comprehensive initial and follow up notes in their functional role as expert clinician and diabetes educator.

6. Acts as a resource to hospital staff for diabetes education and materials. Sets appropriate time aside for insulin, blood sugar management and dietary teaching of the patient in planning for discharge.

7. Evaluates the patient's understanding of their diabetes treatment plan and their ability to self-manage upon hospital discharge. Subsequently provide patient with education on management and equipment needed to manage the disease.

8. Manages patients on the inpatient Diabetes Consult Service. Make rounds with Endocrine fellows and attendings, complete daily patient management assignments and assist with patient care and disposition.

9. After careful examination, data gathering and history taking of the patient,communicates recommendations effectively and in a timely manner with the inpatient primary team.

10. Covers outpatient encounters for other providers when the providers are not available.

11. Ensures proper diabetes follow up appointments with appropriate outpatient provider. If patient is willing, NP will continue to follow patient in the outpatient setting.

12. Demonstrates skill and judgment necessary to direct the medical/nursing plan of care building collegial relationships among the team of his/her colleagues and peers to maximize patient care and outcomes.

13. Delegates responsibilities to others with an understanding of their roles, knowledge and capabilities.

14. Facilitates cooperative and collaborative relationships among the various disciplines and departments to ensure effective quality patient care delivery.

15. Serves as a nursing leader and role model to all team members. Holds self-accountable for professional practice. Presents self in a calm and professional manner.

16. Influences others by establishing a climate for optimal patient care, mutually supportive collegial relationships and professional development.

17. Participates in performance improvement activities utilizing performance improvement principles to support and improve patient care.

18. Attends and actively participates in staff meetings. Accepts responsibility for reading staff meeting minutes when not able to attend.

19. Keeps current with literature regarding changing practices, interventions and research in patient care.

20. Assumes responsibility for seeking out educational and professional opportunities for personal learning needs and growth.

21. Assumes responsibility for meeting mandatory education requirements and unit specific education and competencies.

22. Acts as a preceptor to new NPs and graduate NP students as requested.

23. Maintains collaborative, team relationships with peers and colleagues in order to effectively contribute to the working groups achievement of goals, and to help foster a positive work environment.

Physical Requirements:

1. Prolonged, extensive, or considerable standing/walking

2. Lifts, positions, pushes and/or transfer patients and equipment.

3. Considerable reaching, stooping, bending, kneeling, crouching.

4. Frequent exposure to hazardous chemicals, sick patients, bodily substances, noise and possible exposure to radiation, lasers, electric shock, etc.

5. Regularly exposed to the risk of blood borne diseases and other transmissible infections.

6. Contact with patients under wide variety of circumstances.

7. Subject to varying and unpredictable situations.

8. Handle emergency and crisis situations.

9. May have contact with hazardous materials.

Skills & Abilities:

1. Ability to care for chronically ill patients with a broad range of medical problems.

2. Possess the maturity, self-confidence, and ability to follow complex patient management plans in collaboration with the attending surgeon.

3. Possesses and applies the skills and knowledge necessary to provide care to patients throughout the life span, with consideration of aging processes, human development stages and cultural patterns in each step of the care process.

4. Ability to provide primary care for a caseload of patients according to practice guidelines and hospital policies, procedures and protocols.

5. Leads by example, Positive thinking, focused on goal.

6. Communicates effectively through listening, persuasion, and influence.
