In addition, provides management, oversight and support for nursing staff. The Nurse Manager will also serve as the lead liaison with outside providers, home care and hospice providers. Admission and discharge coordination will be necessary as well.
This position reports directly to the Executive Director. This position interacts with all residents, family members, visitors, and co-workers, and maintains a positive customer service relationship practicing honesty and integrity in all aspects of job performance. In performance of duties, the Nurse Manager will be entrusted with the following responsibilities: In this role, the Nurse Manager, RN will:
- Makes daily rounds to resident rooms to assess clinical and environmental conditions and social/emotional well-being of residents.
- Provides for the management of resident assessment instruments and service plans by ensuring they are up-to-date and accurate.
- Oversees completion of admissions and discharges by licensed nurses and reviews demographic/census data in EMR for accuracy.
- Documents all complaints and concerns. Provides documentation and investigation to the Executive Director. Follows up with resolution.
- Participates in monthly QAPI meetings and follows up with in-servicing and education as needed.
- Provides direction and guidance to the nursing staff in completing assigned care duties and services in accordance with Supportive Living Policies and Regulations.
- Work with designated staff on staff scheduling needs to ensure proper care delivery.
- Manage and oversee the Infection Control program at the community. Manage resident and staff illnesses and keep appropriate records.
- Maintains HIPAA compliance.
- Establishes personal contact with new admissions or transfers and resident families from other levels of care by visiting them in their rooms within 24-48 hours of move in or as specified by state or federal regulations.
- In conjunction with the Executive Director, actively participates in and manages annual state surveys and complaint inspections.
- Responds to voice mail and e-mail messages within 24 hours of receipt or by date requested by sender.
EDUCATION AND WORK EXPERIENCE: Education:
- Bachelor's Degree
- RN License currently licensed in the State
- Currently licensed in good standing as a Registered Nurse (RN) in the State of Illinois.
Experience:
- At least one years' experience in a supportive living community.
- At least one year of management experience required.
- Knowledge in the area of geriatrics.
- Professional training in nursing; have completed specialized training in infection prevention and control as required by state.
- Intermediate level of proficiency in data entry, word processing, email and computer programs.
