Position Summary

This position requires the full understanding and active participation in fulfilling the mission of Saddleback Medical Center. The employee shall support Saddleback Medical Center's strategic plan and participate in and advocate performance improvement/patient safety activities.

The LVN collaborates and coordinates resources with Professional Development to provide duties and responsibilities in response to hospital programs for an assigned patient population.The LVN assists the Professional Development Educator in day-to-day operations related to the patient population. Performs and documents daily in EMR of all assigned patients; assists with patient screening, consent and enrollment in related services as appropriate. Utilizes software and tools designated for documentation of patient care activity. Assists through collaborative approach to develop, implement, and evaluate hospital programs.

Essential Functions

  • Collects and assesses data to perform care interventions based on LVN Scope of Care. Provides input in the formulation of the plan of care. Identifies and prioritizes care based on the patient's individual needs.
  • Meets population and/or department (Professional Development and Age Friendly) specific competencies.
  • Attends and participates in department specific (Professional Development) education/training, in-services, staff meetings, huddles.
  • Completes appropriate documentation in EMR (i.e., consents, plan of care, patient changes, assigned projects, educational activities) in a timely, thorough and accurate manner.
  • Monitors and records adherence of all protocols and program outcome variables. Establishes and addresses reasons for non-adherence and implement changes to improve services.
  • Inputs research study data entry.
  • Provides daily check-ins and support for assigned patients and family members.
  • Assist Professional Development Educator(s) in anticipating and recognizing gaps in daily adherence of protocols.
  • assists with volunteer scheduling, including appropriate patient care assignments.
  • Functions as a member of the clinical teams and delivers related patient care activities.
  • Provides patient and family education as needed.
  • Performs audits as needed.
  • Ability to be at work and be on time.

*Placement in the pay range is based on multiple factors including, but not limited to, relevant years of experience and qualifications. Experience

* Interpersonal and oral communication skills required.

* Data entry and computer skills required, EMR EPIC experience preferred.

* An understanding of medical terminology and experience in a large complex health care setting preferred.

Education

* Current CA Licensed Vocational Nurse (LVN) license required

* Current American Heart Association Basic Life Support (BLS) card required

* Graduate of an accredited Vocational School of Nursing required
