STEMI Program Coord (Accreditation) Job SummaryThe STEMI Program Coordinator plans, develops, organizes, coordinates and facilitates the activities of the hospital's care for the STEMI patient population, which includes education, patient care, problem identification, quality assurance, public relations, hospital-wide standards, statistical data collection and other related responsibilities. EDUCATION Bachelor's degree in Nursing Registered Nurse licensed by the Oregon State Board of Nursing Essential:
1 Coordinates the activities to care for the STEMI population throughout Asante Health System and with external healthcare partners 1.1 Responsible for reviewing STEMI program policies, protocols, and orders; including external EMS and hospital partners. To include yearly site visits to outlying hospitals in Southern Oregon and EMS partners. 1.2 Assists with the development of multidisciplinary training conferences with indicated. 1.3 Conducts case reviews for performance improvement; including STEMI and Non-STEMI readmission reduction fallouts and>10-minute EKG. Provide education on performance improvement. 1.4 Responsible for CARES database entry for OHCA patients arriving at ARRMC and AACH. 1.5 Responsible for ED diversion tracking. 1.6 Documents STEMI activation to determine appropriateness. Perform follow-up and education with EMS and ED. 1.7 Works with EMS directors and Cardiology providers for continuous improvement. 2 Provides leadership role in obtaining Asante's Mission Lifeline Accreditation 2.1 Responsible for Asante's participation and submission for the Mission Lifeline certification. 2.2 Maintain current knowledge of regulatory requirement and readiness for certification surveys. 2.3 Collect and analyze data specific to performance indicators required, developing action plans to correct barriers and to ensure effectiveness. 2.4 Responsible for working with medical directors to develop and maintain evidence-based pathways and efficient processes per guidelines. 2.5 Prepares and distributes annual reports. 2.6 Serves as liaison during surveys. 3 Works cooperatively with internal and external customers. 3.1 Communicates problems to manager and actively participates in problem resolution. 3.2 Gives positive feedback as well as constructive criticism when personally interactive with colleagues. 3.3 Serves as a liaison between Accreditation staff, Administration, Medical Staff, Department Managers, Nursing Staff, Pre-Hospital Services, the community, and other partner organizations. 3.4 Functions as a liaison between Asante STEMI program and other transferring/receiving facilities. 3.5 Excellent communication skills and good working relationship with local EMS. 3.6 Works with EMS agencies to coordinate care and disburse most current data and changes of STEMI care. 3.7 Serves as positive role model and liaison between hospital and EMS community. 3.8 Coordinate with marketing to provide regular community education related to prevention, risk factors, signs, symptoms, and response. 4 Participates in education and performance improvement activities to provide quality service and keep current with changes. 4.1 Maintains current knowledge of practice by attendance at educational events, sharing of education and participation in clinical improvement or research projects. 4.2 Oversees educational opportunities for impacted staff related to changes and ongoing care of STEMI patients. 4.3 Works directly with EMS and Cardiology on quality improvement for STEMI patients. 4.4 Provides outreach education for outlying hospitals in Southern Oregon and gives feedback on transferred patients. 4.5 Participates State of STEMI Talk, assist with data and reporting. 4.6 Presents twice yearly to the STEMI EMS committee. 4.7 Routinely attend and participate in cath conference events and survivor meetings. |
