A Brief Overview
The Clinical Nurse Coordinator is responsible for assuring delivery of safe, efficient, cost effective, age specific nursing care in accordance with CHOP and CHHC standards of practice, policy and procedure and within all regulatory and accrediting agency guidelines. Responsible for and deploys strategies designed to meet defined CHOP clinical, financial and quality initiatives and associated performance and outcome targets. Responsible to coordinate with the patient, caregiver and interdisciplinary team to assure safe and quality patient care to support the plan of care of the home health patient.

What you will do

  • Financial Stewardship
  • Executes and monitors systems to assure consistent and appropriate resource assignment and utilization.
  • Partners with nurse manager to identify and execute quality improvement initiatives that result in a measurable impact on quality, cost and resource utilization.
  • Assures efficient and cost effective utilization of Home Care resources for day to day operations, clinical programming and education.
  • Clinical Excellence
  • Provides competent care for patients and families based on experience.
  • Assures nursing practice and documentation is in accordance with CHOP, CHHC standards of care, policy and procedure as well as all regulatory and accrediting agency guidelines.
  • Participates in on-call rotation to meet patient care needs.
  • Assures regulatory compliance (i.e. environmental, financial, practice, safety, technological, compliance).
  • Partners with Clinical leaders to execute the action plans for measurable continuous improvement in care outcomes.
  • Partners with clinical nurse expert and frontline staff to coordinate all facets of patient care planning with multidisciplinary team.
  • Resources daily operations to ensure nursing sensitive indicator metrics are at or above the mean.
  • Identifies opportunities for improvement and develops an action plan to improve patient outcomes.

Education Qualifications

  • Technical Diploma Nursing Required
  • Bachelor's Degree Nursing Preferred

Experience Qualifications

  • At least two (2) years Previous nursing experience or completion of CHOP Nurse resident program. Required
  • At least one (1) year Leadership experience. Required

Licenses and Certifications

  • Registered Nurse (Pennsylvania) - Pennsylvania State Licensing Board - upon hire - Required or
  • Registered Nurse (New Jersey) - New Jersey State Licensing Board - upon hire - Required and
  • Basic Life Support (BLS) - issued through on organization that requires a hands-on instructor lead psychomotor skills verification (ex. American Heart Association or Red Cross). - upon hire - Required
