Case Manager Children's Outpatient (Full Time)

Location: Woonsocket, RI

Our Children's Outpatient Program is currently seeking a passionate and dedicated Case Manager to work with Children who are impacted by emotional and behavioral concerns. Our goal is to help strengthen the family system by providing support to children/adolescents with an emphasis on social/emotional competency skills and/or daily living skills.

Job Duties:


  • Provide care coordination, resources, advocacy, transportation, and additional supports as needed while building relationships with client and family in office, home and within the community.
  • Collaboration with internal and external providers to enhance level of care (education system, DCYF, Primary Care).
  • Work collaboratively with an integrated team.
  • Maintaining goals and objectives developed by clinical team.

Education/Licensing Requirements:


  • Bachelor's degree in social work, psychology, or other related human services field, with at least one year of experience working with children and families.
  • Must have valid driver's license, reliable transportation, and vehicle insurance.
  • Bilingual applicants are strongly encouraged to apply!
