Job Overview:

Directs the hospital wide Quality, Safety and HRO initiatives. Serves as the lead and coordination efforts for our quality programs, high reliability journey, safety behaviors, and patient experience. Monitors experience scores and assists the VP and hospital leadership in the planning and implementation of tactics, methods and work plans. Identifies areas of opportunity for improvement, champions the development of policies and procedures. Assists leaders in the management and follow up of the patient/family grievance process and ensures timely responses while identifying trends; will serve as an electronic incident reporting administrator. Serves as an internal consultant in education and training for HRO behaviors and encourages employee recognition and engagement. In collaboration with leaders, mentors and grows the safety coach program. Represents NBIMC & CHoNJ as the liaison for quality and experience system meetings and strategies.

Qualifications:

Required:

  • A minimum of a bachelor s degree.
  • At least five years experience in nursing within the hospital setting.
  • Experience with quality improvement, quality management, NJDOH patient safety reporting and HRO behaviors, CPHQ or CPPS preferred, or achieved within two years of hire.

Preferred:

  • Master s degree preferred

Essential Functions:

  • Business Acumen / Operational Effectiveness Assess the learning needs of agency staff in coordination with the Administrative Director based on outcomes from PI. Assist in development of policies and procedures. Assist with reviews of agency processes related to PI outcomes. Responsible for Glucometers, PI and maintenance.
  • Communication Validate with yearly Mantoux for all staff. Participate in hospital committees as deemed appropriate. Responsible for Glucometers, PI and maintenance. The employee demonstrates competence in the following: As appropriate to the ages of the clients served; the ability to obtain and interrupt information in terms of the clients' needs, the clients' growth and development level and demonstrates an understanding of the range of treatment needed by those clients.
  • Leadership Responsible for all PI and Risk Management activities of the Agency and interface with the hospital. Oversees Utilization Review process. Manages/directs the activities of designated agency committees related to PI outcomes. In collaboration with the Assistant Business Manager, facilitates the billing process through evaluation of F.I. denial trends. Responsible for OASIS/OBQI transmission and quality indicators. Oversees Infection Control.
  • People Management Interprets Home Health Policy and Philosophy to physicians, clients, community organizations and the community at large. Educate staff regarding PI activities. Responsible for the Medical Record System and associated staff.
  • Personal Learning / Self-Management Operates with a high degree of autonomy and authority while working within the framework of CMMC s philosophy and policies. Maintains professional affiliations and enhances professional growth and development to keep abreast of the latest trends in hospital/department/administration services.
