Program Manager HIV Outpatient Ambulatory Clinic

PRIMARY FUNCTION:

The Program Manager for the HIV Outpatient Ambulatory Clinic provides administrative and operational oversight of the comprehensive medical and socio-behavioral HIV care program; realizes the achievement of program-related goals and objectives; develops and maintains partnerships with faculty, community organizations, and local government agencies; evaluates and reports Jefferson HIV clinicrsquo;s progress to local and federal funding agencies; and supervises the HIV clinicrsquo;s programmatic and administrative staff.

ESSENTIAL FUNCTIONS:

  • Work with Director of HIV Outpatient Ambulatory Clinic to administer comprehensive HIV program, including ambulatory clinic and network-wide testing, linkage to care, and HIV prevention services.
  • Under guidance of Director, assume lead role in implementing HIV program activities and socio-behavioral service initiatives.
  • Assist Director with grant applications and execute budgets for grants, including purchasing of supplies. Submit continuation of funding applications for program financing.
  • Identify health care payor funds for people living with HIV (PLWH), especially those at highest risk of poor outcomes due to advanced HIV/AIDS. Payors may include Medicaid Fee for Service or Managed Care Organizations, Specialty Pharmacy Benefits Program, Pharmacy Emergency Financial Assistance Program, and Advancing Access or Patient Support Programs.
  • Determine eligibility for Ryan White services and enroll newly diagnosed amp; lost-to-care PLWH into medical care from the emergency department, in-patient hospital service, or external referrals.
  • Serve as liaison and maintain professional relations with the Philadelphia Department of Public Health AIDS Activities Coordinating Office.
  • Develop partnerships and act as a community liaison to promote Jeffersonrsquo;s HIV clinic, including facilitation, recruitment, and coordination of Community Advisory Board.
  • Lead daily operations of the HIV clinic, including program operations funded by local funders, such as, Philadelphia AIDS Fund.
  • Interact with co-workers, visitors, and other staff consistent with Jeffersonrsquo;s iSCORE values.
  • Other duties as requested or assigned.

Bachelorrsquo;s Degree required. Masterrsquo;s Degree in related field, such as Public Health, Social Work, or Population Health, is preferred.

_________________________________________________________________________ CERTIFICATES, LICENSES, AND REGISTRATION:

Current Pennsylvania Social Work license preferred. Certificates for HIV-related knowledge, such as, trauma-informed care, advocacy, and HIV point-of-care testing, preferred.

EXPERIENCE REQUIREMENTS:

Required: At least 3 years of experience in HIV services. Working knowledge of Ryan White Care Act, Medicaid and Medicare regulations as they apply to HIV/AIDS and of other funding for patient care. Experience working on health care service delivery and accessing community-based resources, especially those related to PLWH.

Preferred: Experience in HIV services data management and grants implementation including, but not limited to, health care services delivery, linkage to community services, and/or rapid response to highly complex and vulnerable PLWH.

ADDITIONAL INFORMATION:

Working knowledge of HIV testing informed consent, counseling, and disclosure.

Knowledge of social services and providers, locally and nationally.

Experience with medical case management.

Expertise in CAREware data management.

Working knowledge of electronic medical records, Epic.
