Description
Position Summary:
The Case Manager collaborates with WestCare staff and external bodies to provide assessment, counseling, re-integration, and/or rehabilitation services to program participants. Essential Job Functions include those listed below.
Essential Job Functions:
Provides case management services and assistance to program participants strugglingwith substance abuse, and those in recovery;
Assesses the needs, conditions, and desires of assigned program participants;
Educates and refers program participants to community agencies and businesses thatappropriately address their needs for food, shelter, clothing, employment,transportation, etc.
Assists participants with establishing or re-establishing community support systems;
Acts as advocate on behalf of program participants to improve access to vital services;
Participates in clinical staffing decisions regarding the participants current strengthsand resources, and their need for additional resources;
Completes documentation of services provided in a timely manner;
Provides input into the patients treatment needs and disposition;
Conducts assessments of program participants and provides therapy as needed;
Reviews and documents all clinical services and activities;
Participates in case conferences involving assigned program participants;
Prepares Discharge Summaries, as needed;
Assists other WestCare staff as needed to promote cooperation and portray aprofessional image to the community;
Embraces and embodies the mission, vision, guiding principles, clinical vision and goalsof WestCare Foundation;
Performs any other duties as assigned.
Qualifications
Essential Qualifications:
Certifications/Licensures:
CADC or similar certification preferred; Preference will be given certified candidates;
Valid drivers license and the ability to obtain and maintain WestCares vehicle insurance is required.
Education:
Graduation from an accredited four-year college with a degree in Counseling, Criminal Justice, Social Work, Psychology or other field related to the position is preferred;
High School Diploma or equivalent is required;
Experience and Competencies:
Experience working with individuals suffering from mental health and substance abuse disorders required;
At least two (2) years of experience in counseling, therapy, counseling, and/or social work strongly preferred;
Ability to define problems, collect data, establish facts, and draw valid conclusions required;
Excellent communication skills are required;
Ability to exercise good judgment and discretion required;
Ability to work well in a team environment required ;
Adherence to the highest standard of ethical conduct, especially to standards governing confidentiality required;
Professional appearance and demeanor required;
Must be culturally/linguistically sensitive to populations served;
Intake and assessment experience is highly preferred;
Bilingual ability is preferred;
Prior experience using office equipment, including phones, scanners, printers, and fax machines, and web cameras is highly desired;
