Quality Improvement Facilitator - Northeast Iowa

QI Facilitators are responsible for assessment of provider (HCBS and CNRS) performance, identification of gaps in best practice and making recommendations for improvement methods to improve care, processes and quality outcomes as well as to coordinate and implement quality improvement initiatives.
This position requires residence in the following Iowa counties: Linn, Delaware, Dubuque, Jones, Cedar, Scott, Clinton, and Jackson. Candidates outside these locations will not be considered.

Essential Functions
  • Assess gaps and/or promising practices in HCBS and CNRS provider performance and recommend improvement methods and processes to assist providers and practitioners.
  • Assists in the implementation of the methods and processes.
  • Partners with team members and internal/external customers as necessary, to help determine, evaluate methods, and mentor to encourage participation in the quality initiatives.
  • This may include research of interventions, data analysis, formal and informal education, or presentations.
  • Develops and maintains activity and/or project plans.
  • Ensures plans, tasks, and timelines meet contractual and Company requirements and guidelines.
  • Provides regular updates to Company management on activity and/or project status.
  • Researches and gathers data to support activities and/or project initiatives.
  • Analyze the results of the data collection. Based on the outcome of the analysis, determines areas for improvement.
  • Provides recommendations and feedback to project participants.
  • Assists in the development of project materials and intervention tools.
Requirements
  • Education or experience in a related field such as social work, public health, community development, etc.
  • 3 - 5 years of experience working in direct collaboration with providers, community organizing, population health or public health, and/or health care quality improvement in specific areas as needed and designated by the contract or project.
  • Functional PC knowledge; Experience in specific areas as needed/designated by the contract or project.
  • Up to 50% local or overnight travel as needed.
  • Functional PC knowledge.
  • Must reside in one of the following Iowa counties: Linn, Delaware, Dubuque, Jones, Cedar, Scott, Clinton, or Jackson.
Preferred Knowledge, Skills, and Certifications
  • Health care services, healthcare organizations, and programs
  • Information management processes, electronic health records, and healthcare information technology
  • Quality improvement concepts, principles, and process tools
  • Basic statistics, data collection techniques, data analysis, and graphic representation of data
  • Effective communication both written and verbal
  • Group Process Facilitation
  • QI Process: Focus-Analyze-Develop-Execute/Plan-Do-Study-Act
  • TeamSTEPPS
  • LEAN Six Sigma Green Belt
  • Certified Professional in Healthcare Quality (CPHQ)
  • Community Organizing
