The Resident Care Manager at Cherry Ridge is responsible for the clinical team oversite of The Glen and Rainier Grove. This individual collaborates with the Case Manager to help create a person centered, productive and supportive environment for both residents and staff. The Resident Care Manager is responsible for planning, organizing, and leading the resident care operations through the deployment of St. Ann's Community Mission, Vision and Values. The Resident Care Manager works closely with the interdisciplinary teams to provide compassionate, and relationship based person-centered care and guides the care team in creating a home-like experience. Job Description:
- Assists in creating a person-centered environment for residents and staff through the support of Resident domains of well-being: identity, growth, autonomy, security, connectedness, meaning and joy.
- Analyzes residents/families and staff satisfaction survey data. Involved in the process improvement plans for both areas.
- Monitors compliance related to mandatory in-services, first aid certification, and regulatory requirements as directed by the Administrator. Monitors completion of the RCA and LPN Supervisor annual training requirements, health assessments and nursing license renewals.
- Drives continuous process improvement through initiative and innovation; implements best practices (i.e., audits documentation and monitors process effectiveness).
- Involved in Quality Assurance Performance Improvement projects at the organizational and unit levels.
- Investigates incidents; provides reports to Administrator.
- Collaborates with Case Manager to ensure new admission and annual 3122 completion and accuracy.
- Assists with completion of resident and staff incident reports and is involved in trends analysis.
- Ensures and assists with accurate and efficient documentation. Regularly reviews documentation regarding resident assessments, medical records, and internal documents.
- Coordination of shift-to-shift report and communication pertinent to resident care needs and changes.
- Communicates with area hospitals and rehab facilities related to any resident transfers or returns.
- Performs regularly scheduled in services related to clinical education.
- Collaborates and builds a strong, high performing team, provides coaching in the moment, rounding, team building and mentoring for Resident Care Aides and LPN supervisors.
- Focuses on staff engagement and retention.
- Participates in the overall development of staff members including hiring, orientation, onboarding, professional development, training, discipline and termination of staff in collaboration with both Administration and the Human Resource Department.
- Assists with Monitoring levels of staffing for all (3) shifts, related to unit needs and acuity.
- Approves and monitors time off requests.
- Serves on various committees within St. Ann's, as assigned, and provides reports to Administration on programs and activities, as required.
- Attends and actively participates in workshops, seminars and in-services to continue professional growth.
- Involved in unit-based staff meetings and daily huddles.
- Manager Participates on Duty Weekend Rotation, as scheduled.
- On call requirement to assist LPN Supervisors and RCA's with any issues related to clinical changes and schedule coverage.
- Collaborates with other departments related to unit issues.
- Involved in analysis of workflows and processes; responsible for workflows/process improvement plans.
- Collaborates with Case Manager for unit-based quality improvement. Helps develop and implement unit-based programs.
- Involved in monitoring of career advancement program for care team.
- Collaborates with Administrator, monitors and assures compliance and justification to the annual budget for each unit, identifies and implements cost effective measures, and promotes best practices in standards monthly. Responsible for reporting variances to Administrator.
- Assist with ordering capital items and supplies for nursing units.
- Completes annual staff performance appraisals.
- In collaboration with Case Manager and Administrator responsible for departmental community connection goals.
- Involved in setting annual departmental and individual goals.
Candidate Requirements:
- Current Licensed Practical Nurse (LPN) or Registered Nurse (RN) in good standing.
- Knowledge of assisted living care standards and regulations.
- Supervisory experience required.
- Experience with electronic medical records preferred
