The Behavioral Health Case Manager works with the inpatient behavioral health/medicine multidisciplinary team to provide care for the patients admitted to those units. The BHCM would help with dispositional planning, care planning and group therapy. The BHCM also helps with behavioral health consults in the ED and on the medical unit. Under the supervision and consultation of a psychiatrist the BHCM will gather data about patient condition and assist with disposition, pro viding resources and recommendations to patients in the ED or on medical floors. Performs other duties as assigned.
Responsibilities
Essential Duties and Responsibilities
- Works with patients on the inpatient unit, emergency room and medical floors.
- Consults and coordinates care of patient with the physician/psychiatrist, inpatient RN and other agencies.
- Assesses treatment planning and progress.
- Prepares treatment plan and goals for patients.
- Maintains pertinent patient data in the EMR in accordance with state and federal laws and hospital policy.
- Provides group therapy.
- Arranges for disposition and follow up recommendations.

Education and Experience Master's Degree in a mental health/behavioral health field with documented clinical course work. Experience or field study experience in mental health site. Certification and Licensure TACT certified within 90 days after hire. Approved CPR/BLS within 90 days after hire.
