Position Summary

This position serves as a liaison between Hospice and the Community, Case Managers, Physicians, Patients and Families to ensure continuity of quality care to the hospice patient. This position facilitates & assists in the day-to-day operations of the intake process and is accountable for a smooth admission throughout a patient's/family's transition from admission to the Interdisciplinary Care team. This position will ensure prompt and appropriate delivery of patient care in accordance with physician orders and State & Federal regulations.

This position requires the full understanding and active participation in fulfilling the mission of Saddleback Medical Center. Essential Functions and Responsibilities of the Job

  • Ability to perform Patient Intake & Referral processes and/or the insurance per industry standard.
  • Ability to demonstrate excellent oral/written communication and interpersonal skills as well as active listening.

  • Demonstrate a strong level of honesty regardless of the situation and be sensitive when communicating with others while expressing empathy.
  • Ability to prioritize and multitask
  • Accepts and coordinates Agency referrals in a manner which will foster positive patient, family and referral source satisfaction and provide the most accurate information; demonstrating the highest possible quality of work and efficient in data collection in the EMR system.
  • Demonstrates proficiency at insurance coverage verification, benefit authorization, and reimbursement negotiation with third party payers.
  • Ability to build and maintain relationships with referral sources.
  • Demonstrates leadership in professional activities to promote agency development and personal growth
  • Meets goals for CAHPS survey
  • "Other duties as assigned"

*Placement in the pay range is based on multiple factors including, but not limited to, relevant years of experience and qualifications. In addition to base pay, there may be additional compensation available for this role, including but not limited to, shift differentials, extra shift incentives, and bonus opportunities. Minimum Requirements

Qualifications/Work Experience:



  • One-year practical experience as an LVN and knowledge of home health and or hospice required.
  • Excellent computer skills.
  • Time management and organizational skills, attention to detail, and a focus on completion of tasks, are critical.


Education/Licensure/Certification:



  • Current California LVN license required
