We are looking for a mission driven Social Work Community Health Navigator to join our team! The Social Work Community Health Navigator assesses the social determinants of health and other related needs of patients and connects them to appropriate resources within the community. The position requires a working knowledge of social determinants of health and how they impact the overall health and quality of life of patients. The SW Community Health Navigator serves as a liaison between external community partners and clinical staff members of One World to ensure patients have access to resources and to remove barriers to medical care. This is a full-time position located on our main campus (30th & L). The schedule will be 40 hours per week; generally, Monday-Friday, 8am to 5pm. QUALIFICATIONS:
