A Brief Overview

Children's Hospital of Philadelphia is seeking a Nurse Navigator for their Immunology team.

The Immunology Nurse Navigator is a member of the multidisciplinary team who supports patients with immune system disorders such as allergies, asthma, eczema, autoimmune diseases and primary immunodeficiency disorders. Removes barriers and connects immunocompromised patients and their families to resources and information regarding immune system disorders to help make informed decisions through assessment of the emotional, clinical, financial, psychological and spiritual needs. The Immunology Nurse Navigator serves as a patient advocate, acts as a liaison between the patient and clinical care staff and helps to guide and provide care coordination to patients with immune system disorders and their families.

This position will be Monday-Friday and the shift will dependent upon the operational need. The hiring manager prefers candidates with previous and/or current Nurse Navigator, care coordination, or nurse coordinator experience.

This role serves as the primary clinical contact for patients and families seeking certain specialized services at The Children's Hospital of Philadelphia that require an additional focus on care coordination.

This role is charged with assisting new patients in understanding their care plan, coordinating communication between multiple care team members across the care continuum; participating in family meetings; providing patient/family education, and providing care coordination including scheduling appointments and telephone triage.

The role may also assist with development of outreach and research activities for their department.
Triages/ manages incoming new and established patient calls/email requests.
Assists with the collection and review process for all potential patients.
Works in collaboration with the clinical staff (Medical team, Physical therapy, Social Work, etc.), Out of Area Specialist, Business Offices, to ensure all aspects of the patient and family care coordination results in an ideal patient experience.

What you will do

  • Patient Navigator Responsibilities
  • Coordinate care with referring clinicians and referral patients to simplify access and enhance communication and the patient/family experience at CHOP.
  • Coordinate a plan of care for referral patients and be the person of contact with their referring health care provider to simplify access, enhance communication and the patient/ family experience
  • Coordinate a plan of care for new patient referral and serve as primary contact for referral source (referring health care provider and patient)
  • Collaborate with all appropriate CHOP departments to strengthen relationships and facilitate the referral process
  • Case Management and Care Coordination
  • Assure all pertinent records and studies are available for review; this includes prior testing, imaging and rehabilitation therapies
  • Provide the patient/family and the referring physician with updates and pertinent medical information
  • Patient Scheduling functions: assists schedulers and coordinators in scheduling appointments, both new and follow up, radiology tests, etc.
  • Strategic Planning / Outreach
  • Develop and Implement strategies to increase utilization of CHOP's specialized programs and health care providers within the department where applicable
  • Identify and evaluate new markets, to include statistical analysis of potential markets, estimates of growth, identification of client needs, and competitive analysis
  • Identify, design and implement new strategies that will increase service utilization. Maintain a thorough knowledge of the market and assist in the development of marketing strategies
  • Develop, refine and update referral tools such as patient family education, educational videos and Web Site.
  • Develop and maintain relationships with local School Re-entry Programs, as well as school nurses and other school officials where applicable
  • Network with patients / families through developing a social media support group strategy, organizing support groups, leveraging national and local organizations, and encouraging word of mouth marketing
  • Data Management
  • Develop and maintain physician/referral contact database
  • Capture, track and develop reporting mechanisms on key indicators
  • Help to develop and maintain specialized care registries
  • Help to maintain EMR templates and update as necessary
  • Participate in the triage and possible enrollment of patients into research studies
  • Additional duties as deemed necessary

Education Qualifications

  • Bachelor's Degree Nursing. Employees hired and placed in position previous to 7/28/23 are only required to have technical diploma. Required
  • Master's Degree Preferred

Experience Qualifications

  • At least two (2) years of nursing experience Required
  • At least four (4) years of nursing experience Preferred or
  • Previous experience in pediatrics Preferred

Licenses and Certifications

  • Registered Nurse (Pennsylvania) - Pennsylvania State Licensing Board - upon hire - Required or
  • Registered Nurse (New Jersey) - New Jersey State Licensing Board - upon hire - Required
