Responsibilities

As a member of the treatment team, the Care Coordinator is responsible for coordinating all Recovery Management throughout the continuum of care, directly working with patients coordinating the development of the continuing care plan. Assist in providing all patients with a smooth transition through the Hazelden Betty Ford continuum of care, effectively communicating to families, referents and clients about recovery management.

  • Create a patient/customer focused experience for all seeking Hazelden Betty Ford Foundation services
  • Function as a member of the treatment team by assisting in the continuum of care development.

Qualifications

Required Qualifications:

  • High school diploma or equivalent
  • 2 or more years in customer service
  • Intermediate level experience with Word software, including Outlook, Word, PowerPoint
  • Experience in online research
  • Current CPR certification.
    • Certification must be from American Red Cross or American Heart Association.

Preferred Qualifications:

  • Vocational or associates degree
  • Experience with patient database
  • Basic knowledge of chemical dependency, including Twelve Step abstinence-based treatment programs.
  • Experience in a behavioral care setting
