Central City Concern is an innovative nonprofit agency providing comprehensive services to single adults and families in the Portland metro area who are impacted by homelessness, poverty and addictions. We hire people that are skilled and passionate to meet our mission to end homelessness through outcome-based strategies which support personal and community transformation.

The Blackburn Center is an innovative program that will offer integrated housing, healthcare, and employment services with the long-term goal of eliminating homelessness. We seek to change how these services are delivered to all people, especially vulnerable populations.

The Case Manager works as a member of a multi-disciplinary team providing case management and treatment readiness. The team works to assist clients in addressing all treatment needs, including crisis management, physical health treatment, mental health treatment, accessing benefits and entitlements, accessing appropriate and affordable housing, accessing employment and educational opportunities.

Location: Blackburn Center - 12121 E Burnside St, Portland, OR 97216

Schedule: Monday-Friday 8:00am-5:00pm

Wage: Starts at $21.19/Hour

MINIMUM QUALIFICATIONS:



  • High school diploma or GED and six months of experience working with the homeless population, behavioral health environment, human services, or related environment; OR 6 months of experience in a high-volume customer service role.
  • Understanding of recovery terminology and ability to apply the concepts highly preferred.
  • Must pass a pre-employment drug screen and background check.
  • Will be required to carry an agency cell phone for work use. Cell phone will be provided by Central City Concern.
  • Ability to adhere to Central City Concern's drug-free workplace which encourages a safe, healthy and productive work environment and strictly complies with the Drug-Free Workplace Act of 1988. An employee shall not, in the workplace, unlawfully manufacture, distribute, dispense, possess, or use a controlled substance or alcohol.
  • Must adhere to agency's non-discrimination policies.
  • Must be able to effectively interact with co-workers and clients with diverse ethnic backgrounds, religious views, cultural backgrounds, life-styles, and sexual orientations and treat each individual with respect and dignity.


ESSENTIAL DUTIES AND RESPONSIBILITIES:



  • Provide case management services for clients in recovery.
  • Support clients with setting goals and engaging in activities related to their recovery. Includes detailed case notes and service coordination.
  • Develop strengths-based service plans with client.
  • Participate in clinical staff meetings and case conferences with team members in order to utilize interdisciplinary expertise and facilitate individual service plans.
  • Conduct educational and support groups using approved curriculum.
  • Develop housing plans for Blackburn Stabilization Program clients in preparation for placement to next step Central City Concern transitional housing, residential treatment facilities, or other housing providers.
  • Participate in admission and discharge planning for any client needing hospitalization or sub-acute level of care.
  • Monitor and assure the health and safety needs of clients are met.
  • Use evidence-based practices in addiction and mental health treatment.
  • Coordinate services with all members of client's support network.
  • Refer clients to internal CCC services such as Employment Access, Housing, and Medical and Clinical health.
  • Maintain client records as required by program standards and funding requirements.
  • Assist team with holiday coverage as necessary.
  • Work within scope of training and experience and seek consultation when appropriate.
  • Collaborate with peer support staff to ensure clients have necessary support during meetings and appointments.
  • Adhere to all state and federal privacy regulations, including HIPAA and 42 CFR Part 2, and to CCC policies and agreements regarding confidentiality, privacy, and security. Support compliance with all privacy and security requirements pursuant to community partners' and outside providers' patient confidentiality agreements, including privacy and security requirements for EMR access. This includes immediate reporting of any PHI breach of CCC or outside provider health records to the CCC Legal and Quality departments, as well as to the program administrator.
  • Participate in training opportunities to advance skills and knowledge in addictions, mental health, and criminality.
  • Become certified State of Oregon Enrollment Assister and assist individuals in enrolling for healthcare coverage as directed by supervisor.
  • Perform other duties as assigned.

  • Ability to work in an environment where people may exhibit behaviors that are hostile or abusive.
  • Ability to manage time and meet deadlines.
  • Ability to work courteously and effectively with the general public, clients, medical personnel, corrections personnel, police, and co-workers.
  • Ability to maintain accurate records and necessary paperwork.
  • Apply knowledge of de-escalation methods or ability to be trained in de-escalation methods.
  • Ability to provide leadership.


PREFERRED QUALIFICATIONS:



  • Possess a current driver's license, pass a DMV background check and be designated an "acceptable" driver as outlined in Central City Concern's Fleet Safety Policy. Must pass an initial drivers training within 60 days of being an approved driver and continued recertification training.
