The Clinical Pharmacist ensures the overall integrity, safety, rationality, and effectiveness of the drug use control process for all patients in the assigned geographic or specialty area, including the following patient groups: Neonates, Pediatrics, Adolescents, Adults, and Geriatrics. Licensure, Certification, or Registration Requirements for Hire:
Current licensure with the Virginia State Board of Pharmacy required or obtained within the first 90 days of employment
Licensure, Certification, or Registration Requirements for continued employment:
Licensed as a Pharmacist with the Virginia Board of Pharmacy
Experience REQUIRED: N/A
Experience PREFERRED:
Minimum of one (1) year of pharmacy practice experience in a similar setting or residency training (see below)
Previous experience with state of the art drug distribution and clinical service methods
Education/training REQUIRED:
Bachelors of Science in Pharmacy from an accredited academic program
Education/training PREFERRED:
PGY1, ASHP-accredited residency
PGY2, ASHP-accredited residency
Doctor of Pharmacy (Pharm.D.) from an accredited academic program
Independent action(s) required:
The monitoring, screening, and verification of all drug-related orders for assurance of proper dosage, route, dosage form, and administration schedule.
Ensure expedited preparation and dispensing of all dosage forms by pharmacy personnel.
Resolve drug therapy problems.
Clarify and correct problematic drug orders.
Designate pharmacy technicians to perform special assignments. Evaluate employees' and students' performance.
Counsel patients regarding the safe and proper use of their medications.
Resolve equipment and operational problems. Interact with other health care practitioners in a multidisciplinary environment.
Monitor the effects of medications, and recommend and/or initiate appropriate adjustments to achieve desired medication-related outcomes.
Supervisory responsibilities (if applicable):
Supervise the actions of Pharmacy Interns and Certified Pharmacy Technicians as set forth in the Pharmacy Act and the Drug Control Act with related statutes and regulations of the Virginia Board of Pharmacy.
Supervise the actions of pharmacy residents as set forth in Department of Pharmacy Services.

Must be available to work evenings, nights and weekends.

Transplant Program:

VCU Health's Hume-Lee Transplant Center is renowned for its success in solid organ transplants, including kidney, liver, heart, lung, and pancreas transplants. The center has performed over 4,700 kidney transplants since 1988 and averages around 350 transplants annually. It boasts the shortest average wait time for deceased donor kidney transplants in the region. The center also offers innovative therapies such as living donor exchanges, incompatible kidney transplants, and simultaneous pancreas-kidney transplants.

Role Description:

This is a full-time Clinical Pharmacy Specialist role specializing in Solid Organ Transplant at VCU Health in Richmond, VA. As a Clinical Pharmacy Specialist, you will be responsible for:

  • Providing Pharmaceutical Care: Deliver comprehensive pharmaceutical care for transplant patients (e.g., heart, kidney, liver, lung, pancreas) by assessing patient needs, incorporating age and disease-specific characteristics into drug therapy, and adjusting care according to patient response.
  • Collaborating with Health Care Teams: Work closely with physicians, nurses, and other hospital personnel to optimize medication therapy and ensure integrated pharmacy operational services.
  • Monitoring Drug Therapy Outcomes: Conduct clinical interventions to detect, mitigate, and prevent medication adverse events, ensuring safe and effective medication use.
  • Educating Patients and Healthcare Professionals: Provide education on pharmacotherapy to patients and healthcare professionals, enhancing understanding and compliance.
  • Training and Oversight: Offer educational training and oversight for pharmacy students, pharmacy residents, and other healthcare learners at VCU Health, serving as a preceptor and mentor.
  • Quality Improvement and Research: Participate in and lead quality improvement efforts, including drug use evaluations and clinical research projects in transplant pharmacotherapy. Design and implement stewardship activities and restriction/surveillance programs to improve medication-use processes.
  • Leadership and Liaison: Serve as a departmental resource and liaison to other departments, hospital personnel, or external groups, contributing to practice advancement projects and regulatory compliance initiatives.

Qualifications:

  • Doctor of Pharmacy (Pharm.D.) degree
  • PGY1 and PGY2 transplant pharmacy residency training required or equivalent experience
  • Board Certification: Board certification in pharmacotherapy (BCPS) or a related specialty is preferred.
  • Licensure: Active pharmacy license in the state of Virginia or eligibility for licensure.
  • Skills: Proficiency in Pharmacy Practice and Hospital Pharmacy skills, Medicine and Pharmacotherapy knowledge, and experience with electronic medical records (EMR) systems.
  • Communication: Strong communication and interpersonal skills, with the ability to effectively educate and collaborate with a multidisciplinary team.
  • Leadership: Demonstrated leadership abilities and experience in mentoring and training pharmacy students and residents.
  • Research: Experience in clinical research and quality improvement initiatives.
