Work Schedule:

100% FTE, day shift. 40 hours per week, shifts scheduled Monday through Friday between the hours of 8:30 AM - 5:00 PM. Hours may vary based on the needs of the clinic.

We are seeking a Care Coordination Assistant to:




  • Partner with members of the care team to coordinate patient care, navigate patients through the health care system and provide support for patients receiving specialized behavioral health care.
  • Provide emotional support and education to patients and families within their specialty care discipline.
  • Collaborate with patients and their families to ensure the patient is appropriately scheduled, and facilitate access to clinical, social, and community resources.
