Responsibilities
A hybrid clinical pharmacist primarily focuses on providing direct patient care while also working across multiple clinical and operational settings.
- Monitor drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, allergies and appropriateness of drug and dose.
- Provide accurate and timely clinical consultation and medication information to healthcare staff and patients.
- Document all clinical activities and interventions accurately and completely.
- Sustain the hospital formulary, minimizing non-formulary procurements, utilizing therapeutic interchanges and other approved formulary protocols and/or guardrails.
- Supervise and mentor pharmacy technicians and staff; train and precept pharmacy students/residents.
- Make appropriate evidence-based, patient-centered medication recommendations.
- Participate in the management of medical emergencies as appropriate, including, but not limited to Code Blue.
Qualifications
Job Requirements
- BS. or Pharm D. from an ACPE-accredited School of Pharmacy.
- Clinical pharmacy experience in a hospital setting.
- Completion of an accredited PGY-1 Pharmacy residency or fellowship OR 2+ year's clinical pharmacy experience with no history of residency or fellowship.
- Active Pharmacist license in good standing with the appropriate State Board of Pharmacy.
- Other applicable state requirements - i.e. sterile compounding, etc.
- BLS & ACLS certification (BLS required within 90 days of hire; ACLS required for attending Code Blue).
