A hybrid clinical pharmacist primarily focuses on providing direct patient care while also working across multiple clinical and operational settings.

• Monitor drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, allergies and appropriateness of drug and dose.

• Provide accurate and timely clinical consultation and medication information to healthcare staff and patients.
• Document all clinical activities and interventions accurately and completely.
• Sustain the hospital formulary, minimizing non-formulary procurements, utilizing therapeutic interchanges and other approved formulary protocols and/or guardrails.
• Supervise and mentor pharmacy technicians and staff; train and precept pharmacy students/residents.
• Make appropriate evidence-based, patient-centered medication recommendations.
• Participate in the management of medical emergencies as appropriate, including, but not limited to Code Blue.

Job Requirements

• BS. or Pharm D. from an ACPE-accredited School of Pharmacy.
• Clinical pharmacy experience in a hospital setting.
• Completion of an accredited PGY-1 Pharmacy residency or fellowship OR 2+ year's clinical pharmacy experience with no history of residency or fellowship.
• Active Pharmacist license in good standing with the appropriate State Board of Pharmacy.
• Other applicable state requirements - i.e. sterile compounding, etc.
• BLS & ACLS certification (BLS required within 90 days of hire; ACLS required for attending Code Blue).
