Overview

The Hospice Triage LVN is responsible for assessing, organizing, and directing hospice care within the office setting, with occasional visits to patients as needed. This role requires a skilled nurse with experience in hospice care and community health. The Hospice Triage LVN collaborates with the interdisciplinary team to ensure seamless patient care, responding to triage needs and facilitating appropriate care coordination. This professional nurse uses community resources to develop tailored care plans for patients and families, meeting their needs both within the office and through home visits as required.

Schedule: Saturday & Sunday, 7:45am - 4:15pm

Responsibilities

  • Provide direct patient care to hospice patients, including appropriate pain/symptom management
  • Implement nursing practice following a comprehensive assessment and plan of care
  • Provide accurate and timely documentation of patient services
  • Assess and provide patient and family/caregiver education
  • Participate in coordination of hospice services
  • Provide appropriate pain/symptom management and evaluate patient response to treatments/medications
  • Participate in professional growth and development opportunities and quality assessment activities
  • Ensure compliance with all state and federal legal and regulatory requirements
  • Additional responsibilities as assigned


Qualifications

  • Graduate of an accredited practical or vocational nursing program with current licensure to practice in state of operation
  • 1 year nursing experience required, 3+ years preferred
  • Experience in community health, hospice or med/surg preferred
  • Knowledge of the hospice philosophy of care
  • Commitment to clinical and documentation excellence
