Job Overview: The Hospital Elder Life Program (HELP) is a patient care program that aims to provide extra attention and care to older adults admitted to the hospital. The HELP is a hands-on experience with direct patient contact. The HELP program focuses on high-risk older adults to: Prevent Delirium: Reduce the risk of delirium, which is a sudden state of confusion, Maintain Function: Preserve physical and cognitive abilities during hospitalization, Enhance Independence: Help patients return home as independently as possible and Reduce Readmissions: Prevent unplanned returns to the hospital after discharge. Patients are screened to identify risk factors for delirium, such as dehydration, immobility, sleep deprivation, visual/hearing impairment, or baseline cognitive impairment. Based on the patient's specific risks, interventions are assigned.

In this role you will assist elderly patients under the supervision of the Intervention Clinician or Staff RN to aide in delirium prevention and intervention program as part of the HELP team. This role involves orienting patients and families to the program, implementing intervention protocols, and ensuring adherence to therapeutic activities. This may include but is not limited to: daily orientation -helping patients stay oriented to the time, place, and people around them, early mobilization - getting patients out of bed and active as quickly as possible, sleep enhancement, vision/ hearing adaptations - ensuring patients have access to their hearing aids and glasses, and discharge support guidelines. The role is responsible for evaluating patients to track progress, responding to patient care needs, and updating care plans accordingly. Additional responsibilities include monitoring and documenting intervention protocols and program outcomes, collaborating with interdisciplinary teams to ensure coordinated care, and assisting in the distribution and collection of patient satisfaction surveys. The ideal candidate will enjoy working with the elderly population and their families.

Position Qualifications:

  • High School Graduate required
  • One (1) to three (3) years' working in a related position required
  • Experience supporting elderly individuals strongly preferred
  • Bilingual Spanish strongly preferred

Scheduling Requirements:

  • Full Time, 40 hours per week
  • Evenings, 3pm-11pm
  • Schedule during the week will vary depending on department needs, Weekend and Holidays are required
  • Mandatory New Hire Orientation
