Description
Activa Home Health (an AccordCare Company) is a licensed Medicare-certified home health agency seeking an enthusiastic customer service driven RN Clinical Manager for our Vero Beach Office. This position will primarily work in office and will also perform field case management with responsibilities for assessment, coordination of care with other disciplines and communication with referral sources. Coordinate patient admission with other members of the care team. Must have strong case management experience and working knowledge of Medicare. Responsibilities
