Job Summary and Responsibilities Acentra Health is seeking a full-time Clinical Assessor to join our growing team in New Hampshire. Job Summary As a Clinical Assessor at Acentra Health, you will play a key role in serving your community by conducting comprehensive, needs-based eligibility assessments for individuals applying for Medicaid-funded personal care services. These in-person evaluations-conducted in nursing homes, hospitals, and community-based settings across Nashua, New Hampshire, and surrounding areas-are critical for determining the appropriate level of care and ensuring individuals receive the support they need to thrive. As a Clinical Assessor, you will lead PASRR Level II evaluations that guide care planning, inform placement decisions, and support smooth transitions to the appropriate level of long-term care. Through these essential assessments, you'll play a vital role in promoting patient well-being, improving quality of life, and advocating for services that make a meaningful impact across our communities. This hybrid role offers the flexibility of working from your home office, with local travel throughout Nashua, New Hampshire, and the surrounding areas. All travel expenses, including mileage and parking, are fully reimbursed. Responsibilities

Compensation The compensation for this role ranges from $72,800 to $79,040 in annual base salary.
