Acentra Health is seeking a full-time Clinical Assessor to join our growing team in New Hampshire.

Job Summary

As a Clinical Assessor at Acentra Health, you will play a key role in serving your community by conducting comprehensive, needs-based eligibility assessments for individuals applying for Medicaid-funded personal care services. These in-person evaluations-conducted in nursing homes, hospitals, and community-based settings across Nashua, New Hampshire, and surrounding areas-are critical for determining the appropriate level of care and ensuring individuals receive the support they need to thrive.

As a Clinical Assessor, you will lead PASRR Level II evaluations that guide care planning, inform placement decisions, and support smooth transitions to the appropriate level of long-term care. Through these essential assessments, you'll play a vital role in promoting patient well-being, improving quality of life, and advocating for services that make a meaningful impact across our communities.

This hybrid role offers the flexibility of working from your home office, with local travel throughout Nashua, New Hampshire, and the surrounding areas. All travel expenses, including mileage and parking, are fully reimbursed.

Responsibilities

• Conduct thorough clinical assessments of patients, reviewing relevant records and consulting collateral sources to ensure accurate evaluations and well-informed recommendations.
• Recommend appropriate levels of care and service needs based on a comprehensive evaluation of the patient's physical, behavioral, and psychological functioning.
• Complete assessments promptly and within the scope of practice, meeting all contractual and regulatory requirements.
• Demonstrate strong typing and computer skills, with proficiency in Microsoft Word, Outlook, and other relevant software.
• Adhere to program workflows and ensure compliance with established processes and standards.
• Perform person-centered assessments, providing meaningful and well-considered recommendations for levels of care and support services.
• Leverage clinical expertise and knowledge of psychiatric disorders as outlined in the latest Diagnostic and Statistical Manual (DSM) applicable in New Hampshire.
• Apply knowledge of mental health standards of practice, as well as intellectual and developmental disabilities, geriatrics, and conditions commonly requiring long-term care services and supports.
• Collaborate effectively with healthcare professionals, patients, families, and community support systems to ensure optimal care and outcomes.
• Maintain a high level of attention to detail, prioritize tasks effectively, and execute responsibilities in a timely and efficient manner.
• Communicate clearly and effectively with individuals and groups using verbal, written, and electronic methods.

• Active, unrestricted RN license with eligibility to practice in New Hampshire.
• Graduation from an accredited college or university with a degree in a relevant field, as required for the position. .
• Proficiency in medical record abstracting, including a strong understanding of medical record organization, medical terminology, and disease processes.
• Demonstrated expertise in clinical assessment and critical thinking, ensuring accurate evaluations and recommendations.
• Exceptional verbal and written communication skills, with the ability to convey complex information effectively.
• Proven ability to work collaboratively within a team environment, fostering cooperation and shared goals.
• Flexibility to adapt to changing priorities and strong organizational skills to manage multiple tasks effectively.
• Experience with public mental health and medical programs, as well as public sector services and supports.
• Proficient with Microsoft Office applications, including Excel, Word, PowerPoint, Teams, Outlook, and other web-based software.
• Keen attention to detail, with the ability to prioritize and execute tasks efficiently and in a timely manner.

Compensation

The compensation for this role ranges from $72,800 to $79,040 in annual base salary.

Based on our compensation program, an applicant's position placement in the pay range will depend on various considerations, such as years of applicable experience and skill level.
