Placement Stability, Team Decision Making Facilitator

• Foster Care & Adoption - We reunify and unite families while recruiting and providing support to foster parents and youth in foster care.
• Education & Community Trainings - We help students achieve academic success while giving educators the tools to create safe learning environments to improve their students' behaviors and offer innovative learning opportunities to build and improve knowledge in the community.

We are seeking a Placement Stability, Team Decision Making (TDM) Facilitator to join our team. Your role will ensure service compliance with contract/licensing/funding body/accreditation requirements. You will conduct structured TDM meetings on a flexible full-time basis for all placement related decisions, exercising caution to avoid unnecessary moves and disruptions for children in our care. You will coordinate dialogue between the youth, case management team, members of the support team, placements, relatives, and connected adults to broaden the circle of community and relative support for all placements including relative, foster and group home. As a member of the Kansas Foster Care team, you will work with other team members and report to our Director of Placement Stability.

WHAT YOU WILL DO:

• Facilitate Placement Stability TDMs for ALL situations in which a child is at risk of placement disruption or the time is right to discuss a planned move
• Be immediately available to organize and lead meetings. This position is non-case carrying to remain flexible and facilitate meetings in real time, prior to any child changing placements
• Placement Stability TDM meetings are held to make placement decisions with families. Priority is to hold the TDM BEFORE placement move decision is made
• Attend all required training in accordance with Cornerstones of Care and DCF policy and/or assigned by Cornerstones of Care supervisory staff

WHAT YOU WILL BRING:

Our ideal candidate will have at least 2 years of relevant experience in supervisory level or in the child welfare arena and the following:

• Bachelor's degree in social work or related field
• At least 21 years of age and pass background check, physical and drug screening
• A valid driver's license, proof of current vehicle insurance and reliable transportation

Starting Salary: $48,551 - $53,961 (Annually)
